目的:探讨治疗相关性早幼粒白血病( t-APL)的发病机制、临床及实验室特征。方法:对1例前列腺癌伴发的t-APL行病历分析及血象、骨髓象、FISH等相关检查分析。结果:放疗为本例t-APL的发病原因。血象示三系轻度减少。骨髓象示异常早幼粒细胞克隆性增生。FISH检测证实存在PML/RAR 融合基因并伴发多倍体。结论:t-APL与APL临床及实验室特征相似,而对于骨髓象异常早幼粒细胞比例增高不明显的t-APL,FISH检测为确诊的重要方法。
Objective:To explore the pathogenesis,clinical and laboratory features of the therapy-related acute promyelocytic leukemia( t-APL). Methods:The medical record,results of hemogram,myelogram and fluorescence in situ hybridization( FISH)were analyzed for 1 case of t-APL patient with prostate cancer,and the related literature was reviewed. Results:The radiotherapy was the cause of t-APL. The hemogram showed three series were slightly re-duced. The abnormal clonogenic proliferation of promyelocyte was detected in the myelogram. The FISH confirmed the existence of PML/RAR fusion gene and the gene was associated with polyploidy. Conclusion:The clinical and labora-tory features of the t-APL and APL are similar. FISH is an important method for t-APL diagnosis when the abnor-mal promyelocyte proportion in the myelogram is small.
