The process of enterprise sustainable innovation is full of risk, operator man-made risk plays an important role in that enterprise can achieve sustainable innovation or not. This paper first defines several concepts about operator man-made risk during enterprise’s sustainable innovation, then analyses the internal mechanism and risk characteristics. Finally, in order to improve the completeness of risk identification, it uses the approach of HHM （Hierarchical Holographic Modeling） to identify the risk factors of operator man-made risk of process of enterprise sustainable innovation. It is to be the basis of the next step to assess risk, monitor and manage risk.
