功能性电刺激脚踏车系统在康复医疗中的研究与应用

目的:综合分析功能性电刺激脚踏车系统的研究历史和最新进展,简要介绍目前几种较为成熟的功能性电刺激脚踏车系统,展望该研究领域的发展前景。资料来源:应用计算机检索PubMed和EI数据库1980-0l/2005-01相关功能性电刺激技术脚踏车系统文献,检索词“FES,FNS,FES Cycling”限定文章语言种类为English。同时计算机检索中国期刊全文数据库、维普数据库1995-01/2005-03相关功能性电刺激的文章,检索词“功能性电刺激,脊髓损伤”,限定文章语言种类为中文。资料选择:对资料进行初审,选取研究功能性电刺激脚踏车系统在医疗康复中应用及其系统开发等方面的相关文献。通过摘要来查找全文,纳入标准:是否对患者起到康复作用。排除标准:综述文献、重复研究、Meta分析类文章。资料提炼:共收集到35篇相关文献,20篇文献符合纳入标准,排除15篇。资料综合:20篇文献对功能性电刺激脚踏车进行原理性介绍。涉及功能性电刺激脚踏车的软硬件设计,提出相应的实验、优化设计方法,此外还指出功能性电刺激脚踏车在医疗康复方面起到的作用。结论:功能性电刺激脚踏车训练系统能显著的改善脊髓损伤患者身体状况,甚至将康复医疗器械变为患者的日常工具或者娱乐设施。功能性电刺激脚踏车系统的易于实现性,能给患者带来恢复正常生活的希望。因此,投入更多的精力在功能性电刺激脚踏车系统上是非常有必要的。

英文摘要:

OBJECTIVE: This article reviews the history of functional electrical stimulation (FES) cycling system and its latest development, introduces the several representative FES cycling systems at present, and analyze the developmental prospect in this field. DATA SOURCES: An online search of PubMed and El databases was undertaken by using the keywords of "FES, FNS, FES cycling" to identify the relevant articles published in English between January 1980 and January 2005. Meanwhile, Chinese journals full-text database and VIP information database were scanned with computer to search the Chinese relevant articles published from January 1995 to March 2005, and the keywords were "FES, spinal cord injury" in Chinese. STUDY SELECTION: After the first trial, those articles about the application of FES cycling in medical rehabilitation and the development of FES cycling system were selected. The full-texts were looked up through abstract, and the inclusive criteria was whether the method in the article was helpful for the patients. DATA EX,TRACTION: A total of 35 articles were collected, among which 20 were in accordance with the inclusive criteria and 15 were excluded. DATA SYNTHESIS: The 20 articles, including the design of the software and hardware, introduced the fundamental of FES cycling and the method about experiments and optimizations. Furthermore, these articles showed the effect of FES cycling in the field of medical rehabilitation. CONCLUSION: FES cycling system can greatly improve the overall fitness and health of spinal cord injury. In a way, it becomes a recreational device, rather than a simple medical ergometer. And FES cycling system is easy to realize, which can make people have confidence of rehabilitation. Therefore, it is very necessary to make more effort to FES cycling research.
