目的采用计算机辅助方法,利用实时三维超声心动图获取共同房室瓣环的三维定量运动信息,以期辅助房室间隔缺损手术规划和疗效的定量评估。方法①采集出共同房室瓣环上的边界特征点;②对边界特征点按闭合环状排序;③使用三维非均匀有理B样条算法拟合点环重构瓣环;④对共同房室瓣环进行动态分析。结果得到了共同房室瓣环较准确的三维形态变化图,计算出一个心动周期内共同房室瓣环时间序列上的近似面积和高低点间的距离。在心脏舒张末期,术后二尖瓣环与术前共同房室瓣环的投影面积比值与左侧房室瓣的反流情况相关性良好。结论本方法对了解完全型房室间隔缺损的病理特征和施行房室瓣环成形术的效果评价有辅助作用。
Objective To assess 3D quantitative motion information of the common atrioventricular ring by RT3DE with a computer aided method,in order to support atrioventricular septal defects'surgical planning and quantitative clinical assessment.Methods The method consists of the following four steps:①mark the boundary points on the common atrioventricular ring;②re-arrange these marked points into a series of points on a closed contour;③describe the common atrioventricular ring by the 3D non-uniform rational B-spline algorithm;④calculate the new parameters related to the dynamics of the common atrioventricular ring throughout the cardiac cycle.Results This approach successfully determined unique 3D descriptors of the common atrioventricular ring's geometry during a cardiac cycle.The largest projected area,the distance between the highest point and the lowest point of the common atrioventricular ring were measured.In the end of the diastole,the proportion of the mitral annulus after surgery to the common atrioventricular ring before surgery in the projected area had a good correlation with the left atrioventricular valve regurgitation.Conclusion This method has certain reference value in clinical diagnosis of atrioventricular septal defects and atrioventricular central angioplasty.
