目的 探讨长脉冲Gentle YAG 1064 nm 激光治疗龟头浅表静脉畸形的临床疗效及安全性.方法 对12例经MRI确诊为浅表龟头静脉畸形患者使用长脉冲Gentle YAG 1064 nm激光进行治疗,术中动态冷却,光斑直径1.5 mm或3.0 mm,脉宽10-30 ms,能量密度180 - 260 J/cm^2,治疗间隔1.0-1.5个月,总治疗次数2-8次.疗程结束后随访并进行医师评价、患者满意度的5级评价及并发症记录.结果 疗程结束后1个月,医师根据疗效评价:5级5例,4级7例;患者满意度评价:8例非常满意,4例满意.随访3 -18个月,均未见复发.1例患者于第1次治疗后轻微感染,2周后愈合,并残留浅表瘢痕;所有患者治疗后均无出血、色素改变及勃起功能障碍.结论 对于浅表龟头静脉畸形应用长脉冲Gentle YAG 1064 nm激光治疗效果肯定,安全性高.

英文摘要:

Objective To investigate the therapeutic effect and safety of the long-pulse 1064 nm Gentle YAG laser in treatment of superficial venous malformations of the glans penis. Methods Twelve patients with superficial venous malformations of the glans penis were treated with long pulsed 1064 nm Gentle YAG laser. Correlated parameters are as follows : energy density 180 - 260 J/cm^2, spot diameter 1.5 mm or 3.0 mm, pulse width 10 to 30 ms. All of the patients were treated at a 1 to 1.5 months interval and followed up to evaluate the therapeutic efficacy and record side effects. Results The physicians' assessed degrees of improvement were excellent in five patients, significant in seven. And the patient's satisfaction surveys showed that 8 cases were very satisfied, 4 cases were satisfied. 3 to 18 months of follow-up showed no recurrence. A mild infection was happened on 1 patient after the first treatment and healed in 2 weeks with superficial scars. All patients had no bleeding, pigmentation and erectile dysfunction. Conclusion The long-pulsed 1064 nm Gentle YAG laser is effective and safe in the treatment of superficial venous malformations of the glans penis.
