目的:了解天津地区葡萄球菌的耐药状况、分子分型及遗传学特点.方法:受检菌株94株,抗生素药敏试验测定;应用PCR及多重引物PCR技术对耐甲氧西林葡萄球菌中的葡萄球菌盒染色体mec(SCCmec)以及盒染色体重组酶基因(ccr)进行基因分型;8株耐甲氧西林金黄色葡萄球菌(MRSA)进行脉冲场凝胶电泳分型;6株做多位点测序分型(MLST).结果:8株MRSA对包括苯唑西林在内的多种抗生素耐药,但MRSA和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)对糖肽类和新的抗革兰阳性菌药物均敏感;天津地区MRSA多含有Ⅲ型SCCmec,MIST分型以ST239为主;MRCNS中的SCCmec类型复杂,用现有的方法大多不能分型.结论:天津地区葡萄球菌中耐药情况较为严重,本地区MRSA所携带的SCCmec并非来自:MRCNS,而MRCNS含有更为复杂的SCCmec.
Objective: To understand the antibiotic resistance, molecular typing and genetic characteristics of Staphylococcal strains in Tianjin area. Methods: Ninety-four staphylococcal strains were examined by antibiotic susceptibility test. SCCmec types and ST types were tested by muhiplex-PCR and multi-locus sequencing typing (MLST), 8 methicillin resistant Staphylococcus aureus(MRSA)were tested by pulse field gel electrophoresis(PFGE)and 6 MRSA were tested by MLST. Results: Eight MRSA strains were resisted to almost all kind of antibiotics. MRSA and MRCNS were susceptible to glycopeptide. Most of MRSA strains harbored SCCmec type Ⅲ and belonged to ST239. MRCNS had more complicated SCCmec elements which could not be typed by current methods. Conclusion: The situation of antibiotic resistance of Staphylococcal isolates is serious in Tianjin area. No extensive interspecies SCCmec transfer between MRSA and MRCNS. MRCNS seems to harbor more comphcated types of SCCmec.
