目的:探讨血浆炎性因子与进展性脑梗死之间的相关性。方法选择2013年8月-2015年8月在我科治疗的100例脑梗死患者,根据病情的发展分为两个亚组:稳定型脑梗死组和进展性脑梗死组,检测并比较两组血浆(interleukin-6,IL-6)、(interleukin-8,IL-8)和(tumor necrosis factor-alpha,TNF-α)水平差异;根据病史、体检、无创性检查及脑梗死诊断和分级标准,将进展性脑梗死患者严重度分级为重度、中度和轻度,比较稳定型脑梗死组和进展性脑梗死组两组之间及进展性脑梗死不同等级之间IL-6、IL-8和TNF-α水平。结果与稳定型脑梗死组相比,进展性脑梗死组IL-6、IL-8表达水平明显高,有统计学差异(P<0.05);TNF-α表达水平两组无明显差异(P>0.05);采用配对四格表的卡方检验( McNemar检验)进行分析,结果显示重度、中度和轻度分组之间,IL-6、IL-8和TNF-α表达水平存在两两差异(P<0.05),并随着病情的加重逐步升高。结论血浆炎性因子变化与进展性脑梗死之间存在一定的关联性,炎症反应可能参与了脑梗死进展的病理生理机制。
Objective This paper is intended to investigate the correlation between the changes of plasma inflamma -tory factor levels and progression of cerebral infarction .Methods 100 cerebral infarction patients hospitalized in our de-partment from August,2013 to August,2015 were enrolled in this study.Patients were divided into two groups,basing on the progression of the conditions of the cerebral infarction ,the stable cerebral infarction group and progressive cerebral infarction group.Serum concentrations of IL-6 and IL-8 and TNF-αwere measured and compared between two groups .According to the history,physical examination,non-invasive examination and diagnosing and classification criteria of cerebral vascular dis -ease,progressive cerebral infarction were classified as mild ,moderate and severe in severity .The levels of serum IL-6,IL-8 and TNF-αwere compared among the three subgroups ,mild,moderate and severe subgroups .Results Compared with the stable cerebral infarction group ,IL-6 and IL-8 levels were significantly higher in the progressive cerebral infarction group ( P〈0.05) ;but there was no significant difference in TNF-αlevel between the two groups (P〉0.05).Paired fourfold table card party test (McNamara test) for data analysis showed significant differences in IL-6 and IL-8 and TNF-αlevels among the mild,moderate and severe subgroups of progressive cerebral infarction group ,and the levels of IL-6 and IL-8 and TNF-αwere positively related with the severity of the progressive cerebral infarction (P〈0.05).Conclusion Serum inflamma-tory factors changes are correlated with the progression of cerebral infarction ,and inflammatory responses may take part in the pathoohysiology of progressive cerebral infarction .
