目的:探讨超声弹性成像与磁共振动态增强成像在乳腺肿瘤诊断中应用价值。方法:收集2010年∽2013年来我院经临床及病理诊断为乳腺肿瘤140例患者的影像资料,其中乳腺癌62例,良性肿瘤78例,且术前均行动态增强及弹性成像检查,将所得图像信息进行分析,并与病理结果比较,分别获得弹性成像、动态增强及两者结合对乳腺癌诊断的敏感性、特异性、准确性。结果:所有病例均病理证实,磁共振动态增强检查诊断乳腺癌的敏感性为79%,特异性80.8%,准确性80%;超声弹性成像检查诊断乳腺癌的敏感性77.4%,特异性78.2%,准确性77.9%;两种方法结合检查诊断乳腺癌的敏感性91.9%,特异性91%,准确性91.4%;弹性成像、动态增强检查诊断乳腺癌结果比较无统计学差异(P〉0.05);弹性成像、动态增强与两者联合的诊断结果比较有显著性差异(P〈0.05)。结论:磁共振动态增强和超声弹性成像结合对乳腺癌诊断的敏感性、特异性、准确性均明显高于单独一种检查。
Objective: To explore the value of ultrasound elastography and dynamic-enhanced magnetic resonance imaging in diagnosing breast tumors. Methods: The image data of 140 breast tumor cases,which were diagnosed by clinical and pathological,were collected. Among them,62 cases of breast cancer and benign tumor in 78 cases. Dynamic enhancement and elastography had been done to all the cases before surgery. All the image data were analyzed and compared with pathology to get the sensitivity,specificity and accuracy of elastography,dynamic enhancement and a combination of both. Results: All cases were confirmed by pathology. The sensitivity of the diagnosis of breast cancer with dynamic-enhanced magnetic resonance imaging was 79%,specificity 80. 8% and accuracy 80%; the sensitivity 77. 4%,specificity78. 2% and accuracy 77. 9% of ultrasound elasticity imaging in the diagnosis of breast cancer. While,when ultrasound elasticity imaging was combined with dynamic enhanced,the sensitivity of the diagnosis of breast cancer was 91. 9%,specificity 91% and accuracy 91. 4%. When the results of the diagnosis of breast cancer of elasticity imaging and dynamic enhancement examination were compared with each other,there was no statistical difference( P 0. 05). When the diagnostic results of Elasticity imaging,dynamic enhancement and the combined two,there was significant difference( P 0. 05). Conclusion: The sensitivity,specificity,and accuracy are significantly higher than those of any single check when the diagnosis of breast cancer is done by combining dynamic contrast-enhanced magnetic resonance imaging and ultrasound elastography.
