目的评价急性脑梗死患者血管内皮功能的变化及其与危险因素的关系。方法回顾性连续纳入2013年4-7月在首都医科大学附属北京朝阳医院神经内科就诊的患者81例,根据头部MRI结果,分为急性脑梗死组(57例),同期经头部MRI以及腰椎穿刺等检查,排除脑梗死和脑炎等神经系统疾病的头痛、头晕患者为对照组(24例)。急性脑梗死发病72h内进行内皮功能一外周动脉张力测定技术(Endo-PAT)检测,以反应性充血指数(RHI)作为主要指标,评价急性脑梗死患者血管内皮功能情况。采用多重线性回归分析RHI与性别、年龄、吸烟史、急性脑梗死、血中C反应蛋白及肌肝的相关性。结果①Endo-PAT检测结果,急性脑梗死患者RHI平均为2.02.4±0.40,对照组为2.63±0.21,两组比较差异有统计学意义(P〈0.05)。②多重线性回归分析显示,RHI与性别、年龄、急性脑梗死(r^2=0.969)、吸烟史、C反应蛋白水平和血肌酐水平相关,P均〈0.05。结论急性脑梗死患者血管内皮功能减退,RHI水平与性别、年龄、吸烟史、血中C反应蛋白及肌酐水平相关。
Objective To assess the changes of blood vessel endothelial function in patients with acute cerebral infarction and its relationship with risk factors. Methods A total of 81 consecutive patients treated at the Department of Neurology, Beijing Chaoyang Hospital, Capital Medical University from April 2013 to July 2013 were enrolled retrospectively. According to the results of head MRI, they were divided into an acute cerebral infarction group ( n = 57), and over the same period, the patients with headache and dizziness but excluded cerebral infarction, encephalitis, and other nervous system diseases were used as a control group (n = 24). Endothelial peripheral arterial tonometry (EndoPAT) was performed within 72 hours after acute cerebral infarction. The reactive hyperemia index (RHI) was used as a main indicator to assess the endothelial function of the patients with acute cerebral infarction. Multiple linear regression analysis was used to analyze the correlations between RHI and sex, age, acute cerebral infarction, smoking history, C-reactive protein and serum creatinine levels. Results ①The results of EndoPAT test showed that mean RHI was 2.02 ± 0.40 in patients with acute cerebral infarction, and it was 2.63 ±O. 21 in the control group There was significant difference between the two groups (P 〈 0.05 ). ② Multiple linear regression showed that RHI was correlated with sex, age, acute cerebral infarction( r^2 =0. 969), smoking history, C-reactive protein and serum creatinine levels (all P 〈 0.05 ).Conclusion Endothelial function decreased in patients with acute cerebral infarction. RHI level is associated with sex, age, smoking history,C-reactive protein and creatinine levels.
