目的 探讨在超声乳化治疗晶状体半脱位手术中，采用中期提前植入人工晶状体替代囊袋张力环方法的可行性。方法对16例（17只眼）晶状体半脱位患者行超声乳化白内障吸出联合人工晶状体植入术，术中在超声乳化吸出部分晶状体核后提前植入人工晶状体（部分患者酌情缝线固定），替代囊袋张力环，然后继续行超声乳化白内障吸出及人工晶状体植入术，观察手术疗效。结果所有17只眼均顺利完成手术。手术后15只限（88．24％）的人工晶状体均处于正位，2只眼（11．76％）轻微倾斜。手术后患者最佳矫正视力较手术前明显提高（x^2=13．24，P〈0．05），其中〉0．8者6只眼，0．4～0．7者6只眼，0．1—0．3者3只眼，0．1以下者2只眼。术中及术后未见严重或特殊并发症。结论在超声乳化治疗晶状体半脱位的手术中，采用中期提前植入人工晶状体替代囊袋张力环的方法是安全有效的，能提高手术安全性，防止人工晶状体偏位，减少手术并发症，降低手术成本及植入异物的风险．
英文摘要：
Objective To study the Surgical treatment of subluxated lens by intraocular lens （ IOL ） implantation shift to an earlier time replacing capsular tension ring （ CTR） during phacoemulsifieafion. Methods 17 eyes with subluxated lens was performed by IOL implantation shift to an earlier time replacing CTR during phacoemulsification. The visual acuity , the position of IOL , and the postoperative complications were observed and analyzed. Results All 17 eyes received surgery sueeessfuUy. After operation, 15 lens （ 88.24% ） were on the fight position and only 2 lens （ 11.76% ） were slightly tilting. Best corrected visual acuity （ BCVA ） with all 17 eyes postorerafiveily improved significantly compared to preoperation（ X^2 = 4.85, P 〈 0.05） ： 6 eyes over 0. 8,6 eyes with 0. 4 -0. 7,3 eyes with 0. 1 -0. 3,2 eyes under 0. 1. There were no serious complications during or after the operation. Conclusion It is safe and efficient to surgery of subluxated lens by intraocular lens implantation shift to an earlier time replacing capsular tension ring during phacoemulsitlcation.
