目的 探讨帕金森病(Parkinson's disease,PD)患者血清碱性磷酸酶和血清总钙水平变化和骨代谢的相关性.方法 选择2009年1月~2013年12月予江苏省人民医院神经内科门诊和病房就诊的286例PD患者为PD组,选择年龄、性别、民族与PD患者相匹配的健康人290名为对照组.检测两组受检者的血清碱性磷酸酶和血清总钙水平并进行比较.结果 PD组血清碱性磷酸酶水平为(80.30±20.62)U/L,对照组为(83.49±15.84)U/L,两组血清碱性磷酸酶水平比较差异有统计学意义(P< 0.05);PD组血清总钙水平为(2.26±0.10)mmol/L,对照组为(2.37±0.06) mmol/L,两组血清总钙水平比较差异有统计学意义(P<0.05).不同性别人群血清碱性磷酸酶和血清总钙浓度间差异有统计学意义(P<0.05).在PD组的不同分期中,H-Y1、H-Y2、H-Y3患者和对照组比较,血清碱性磷酸酶水平差异有统计学意义(P<0.05).血清总钙水平在H-Y分期1~5患者中与对照组比较差异均有统计学意义(P<0.05).61~<71岁的PD患者血清碱性磷酸酶水平与同年龄对照组比较,差异有高度统计学意义(P<0.01).PD组中血清总钙水平在所有年龄段与同年龄段对照组比较,差异均有统计学意义(P<0.05).结论 PD患者的血清碱性磷酸酶和血清总钙水平均低于健康者,PD患者存在骨代谢异常,碱性磷酸酶和血清总钙水平可能作为PD的诊断指标.
Objective To examine the correlation between serum alkaline phosphatase (ALP), serum calcium (Ca) of patients with Parkinson's disease (PD) and bone metabolism. Methods 286 patients with Parkinson's disease in Outpa- tient Clinic and Ward of Neurology Department, Jiangsu Province Hospital were selected from January 2009 to Decem- ber 2013 as PD group, 290 healthy persons were enrolled as control group, the age, gender and nation between the two groups were matched. The relationship between the levels of serum ALP and serum Ca was studied. Results The level of serum ALP in PD group was (80.30±20.62) U/L, which in the control group was (83.49±15.84) U/L, the difference was statistically significant (P 〈 0.05). The level of serum Ca in PD group was (2.26±0.10) mmol/L, which in the control group was (2.37±0.06) mmol/L, the difference was also statistically significant (P 〈 0.05 ). At the same time, the differ- ences of serum ALP and serum Ca between different gender groutis had statistically significant (P 〈 0.05). In the differ- ent stages of PD group, serum ALP level between H-Y1, H-Y2, H-Y3 stages of PD patients and control group had sta- tistically significant differences (P 〈 0.05). Serum Ca levels in each H-Y stage had statistically significant differences compared with control group (P 〈 0.05). Serum ALP level in 61-〈71 years old PD patients had statistically significant difference compared with patients with same age in the control group (P 〈 0.01). Serum Ca levels in all age stages PD patients had statistically significance differences compared with the control group (P 〈 0.05). Conclusion Serum ALP and serum Ca levels of PD patients are lower than the normal people, which may involve in bone metabolic abnormali- ties. Serum ALP and serum Ca levels are expected to be used as diagnostic index of PD.
