目的 观察Alberta评分联合婴幼儿语言评定量表在脑性瘫痪(脑瘫)患儿康复训练中的应用。方法 选择2012年6月至2013年6月在苏州大学附属儿童医院神经康复科进行治疗的脑瘫患儿64例,根据干预方法不同采用随机数字表法分为观察组和对照组各32例。对照组行常规康复训练,包括运动疗法(主要是Bobath疗法、上田疗法)及应用婴幼儿语言评定量表进行治疗;观察组在此基础上应用 Alberta评分进行评估,根据Alberta婴儿运动量表(AIMS)评估结果设定动作、制订康复计划。每天坚持3 h,反复强化训练。比较2组患儿治疗后的疗效及2组患儿治疗前后大动作及精细动作发育商(DQ)的变化情况。结果 观察组和对照组治疗后的总有效率分别为90.6% 和71.9% ,2组总有效率比较差异有统计学意义(χ^2=6.317,P 〈0.05)。观察组患儿治疗后的大动作(47.92±7.15)及精细动作 DQ(42.55±8.13)均较治疗前(36.18±8.23,33.71±10.16)及对照组治疗后(38.13±8.21,36.58±8.06)明显升高,差异均有统计学意义(t=6.235、5.452、6.137、5.243,P均〈0.05)。结论 在脑瘫患儿康复训练中结合Alberta评分联合婴幼儿语言评定量表,有利于提高康复治疗效果,改善患儿的运动功能,值得推广和应用。
Objective To observe the Alberta combined with Infant Language Assessment Scale used in rehabilitation for children with cerebral palsy. Methods From Jun. 2012 to Jun. 2013,64 cases of cerebral palsy in Children's Hospital Affiliated to Suzhou University were selected according to the different interventions and were randomly divided into an observation group and a control gToup,32 patients in each group. The control group underwent conventional rehabilitation training included exercise therapy (mainly Bobath therapy, Ueda therapy) and application Infants Language Assessment Scale for treatment, and the observation group on the basis of the method above, set the action in accordance with Alberta infant motor scale (AIMS) assessment to develop rehabilitation programs. Adhere to 3 hours a day of repeated intensive training. Efficacy in children after treatment was compared, and forceful nmve and fine motor movements and the changes in development quotient(DQ) before and after treatment were compared between 2 groups. Results The total efficiency of the observation group and the control group was 90.6% and 71.9% , respectively, and there was statistically significant differences between 2 groups (χ^2 = 6.317, P 〈 0.05 ). After treatment, the DQ of big movement and fine motor in observation group(47.92 ± 7. 15,42.55 ± 8. 13 ) were significantly higher than before treatment(36.18± 8.23,33.71 ± 10.16) and the control group (38.13 ± 8.21,36.58 ± 8.06 ), the differences were statistically significant( t = 6. 235,5. 452,6. 137,5. 243, all P 〈 0.05 ). Conclusions The combination of the rehabili- tation for children with cerebral palsy in infants language Alberta combined score rating scale, help to improve rehabilitation results and motor function in children, which is of recommendation and application.
