目的对2例微柱凝胶卡漏检BwO3亚型标本血型血清学特点及其分子遗传背景进行系统分析,以期为临床安全输血提供帮助。方法分别应用试管法和微柱凝胶卡法进行血型正反定型;应用PCR—SSP方法对ABO血型基因进行初步分型;并进一步应用直接测序和单体型序列分析法对ABO基因第6、7外显子进行分析以确定其基因型。结果标本1:试管法结果疑似B亚型,凝胶卡结果正定为O型反定为B型,基因检测结果为BwO3。标本2:试管法结果疑似AB亚型,凝胶卡结果正定为A型反定为AB型,基因检测结果为ABwO3。结论微柱凝胶卡能漏检BwO3亚型,应哭用多种方法进行血型鉴定以减少临床血型鉴定错误及相关输血反应的发生。
Objective To analyze the serological characteristics and molecular genetic background of two Bw03 subtypes falsely detected by microcolumn gel cassette assay, in order to provide assistance for clinical blood transfusion. Methods Tube test and microcolumn gel cassette assay were used to detect the positive and negative stereotypes respectively. PCR-SSP was applied to identify the genotypes of the two samples. The sequence analysis of exons 6 and 7 of ABO gene was performed by sequence-based testing and cloning sequencing. Results The result of sample 1 from tube test was possibly B subtype. The results of positive determination was O and negative determination was B. Genetic test result was B.03. In sample 2, the result from tube test might be AB subtype. The results of positive determination was A and negative determination was AB. The genetic test result was ABw03. Conclusion B,03 can be falsely detected by microcolumn gel cassette assay. Various methods should be adopted to identify the correct type to reduce clinical blood typing errors and related blood transfusion reactions.
