目的探讨B淋巴细胞微粒(辟LMPs)在类风湿关节炎(RA)发病机制中的作用。方法采用流式细胞术检测39例活动期RA患者(RA组)和24例健康体检者(健康对照组)外周血B-LMPs含量,检测RA患者外周血CRP、ESR、RF、ACPA指标,分析B-LMPs含量与各指标相关性。其中20例有膝关节积液RA患者(关节液组)同时检测配对外周血和关节液B-LMPs含量。结果RA患者外周血B-LMPs含量显著高于健康对照组(P〈0.01);RA患者关节液昏LMPs含量明显高于配对外周血(P〈0.05)。RA患者外周血B-LMPs含量与疾病活动性评分DA$28呈正相关(r=0.5431,P=0.0009),与ESR、CRP、RF、ACPA无相关性;而关节液BLMPs含量与疾病活动性评分DAS28呈负相关(r=-0.470,P=0.0362),与CRP、ESR、RF、ACPA无相关性。结论RA患者外周血BLMPs表达异常,且与疾病活动性显著相关,提示&LMPs在RA疾病活动和关节损害中发挥重要作用。
Objective To test the expression of B-lymphocyte Microparticles (B-LMPs) in rheumatoid arthritis (RA) patient's plasma and synovial fluid and compare the difference between patients and healthy controls (HC). Methods The levels of B-LMPs in plasma were detected by flow eytometry in 39 active RA patients and 24 healthy volunteers. Synovial fluid (SF) was collected from 20 patients. Clinical parameters including CRP, ESR, RF, ACPA in the patients and volunteers were observed and analyzed. The potential correlation of B-LMPs counts and clinical parameters was analyzed. Results The B-LMPs levels in SF from RA patients were higher than paired plasma (P〈0.05) and in plasma from HC (P〈0.01). The levels of plasma B-LMPs were positively correlated with Disease Activity Score DAS28 (r=0. 5431, P=0. 0009) and not correlated with ESR, CRP, RF and ACPA. B-LMPs levels in SF were not correlated DAS28, ESR, CRP, RF and ACPA (P〉0.05). Conclusion B-LMPs may be involved in immune regulation and systemic inflammation of RA. The elevated levels of PMPs could be a potential biomarker for RA.
