目的探讨紫绀型先心病缺氧是否致脂肪组织缺氧及内分泌功能紊乱。方法选取紫绀型先心病(CCHD)和非紫绀型先心病(ACHD)手术患者共32例,其中CCHD 17例为实验组,ACHD 15例为对照组,比较身高、体重、血红蛋白。获取皮下脂肪组织,免疫组化检测缺氧诱导因子1(HIF-1α),RT-PCR法分析脂肪因子等表达,Western blot法检测P-NFκBP65、P-IκBα、JNK和P-JNK蛋白表达。结果实验组与对照组相比,身高(67.1±7.8)cm vs(72.6±9.5)cm(P〈0.05);体重(7.9±2.2)kg vs(9.5±2.6)kg(P〈0.05);血红蛋白(186.6±22.6)g/L vs(117.4±10.5)g/L(P〈0.05)。实验组HIF-1α、内质网应激标志和炎症相关指标表达高于对照组(P〈0.05)。结论紫绀型先心病导致脂肪组织缺氧,脂联素表达下降,炎症因子增加,其变化可能与内质网应激相关。

英文摘要:

[Objetive] To investigate whether hypoxia in cyanosis congenital heart disease (CCHD) caused adipose tissue hypoxia and endocrine dysfunction. [Methods] 32 patients were divided into two groups: 17 cases in cyanosis congenital heart disease (CCHD) as experimental group, and 15 cases in Acyanosis congenital heart disease (ACHD) as control group. Height, weight and hemoglobin were compared. Subcutaneous adipose tissues were collected. Im- munohistochemical staining was used to detect HIF-lc~, the mRNA expression of adipokines and the protein expres- sion of P-NFKBP65, P-IKBc~, JNK and P-JNK were detected by using RT-PCR and Western blot. [Results] Ex- perimental group compared with the control group, the height was (67.1+7.8)vs.(72.6-+9.5)cm (P 〈0.05); The weight was (7.9+2.2)vs.(9.5+2.6)kg (P 〈0.05); The hemoglobin was (186.6+22.6)vs. (l17.4_+10.5)g/L (P 〈0.05). The expres- sion of HIF-lc~, marker of endoplasmic reticulum stress and inflammatory pathways were increased, which compared to the control group (P 〈0.05). [Conclusion] Hypoxia in CCHD caused adipose tissue hypox~a, the expression of adiponectin was decreased and inflammatory factors were increased, these change may were related to endoplasmic reticulum stress.
