目的通过观测不同临床分期骨巨细胞瘤组织中低氧诱导因子(HIF)及其下游靶基因血管内皮生长因子(VEGF)表达水平,了解HIF在骨巨细胞瘤发病中的凋节作用。方法收集2006年6月至2009年7月间18例骨巨细胞瘤病例,根据Campanacci骨巨细胞瘤影像学临床分期进行分期;采用免疫组织化学技术分别对18例骨巨细胞瘤组织标本进行HIF-1α、HIF-2α和VEGF染色,观测其表达水平及分布状况。结果18例患者中5例为Ⅰ期病损,4例为Ⅱ期病损,5例为Ⅲ期病损,4例为复发病损。组织标本切片免疫组织化学染色显示,有7例(7/18)呈HIF-1α阳性反应,全部标本呈HIF-2α、VEGF阳性反应;HIF-1a表达水平在各期病例组间无明显差异(P〉0.05),HIF-2α、VEGF表达水平在Ⅱ、Ⅲ期和复发病例组明显高于Ⅰ期病例组(P〈0.05)。结论骨巨细胞瘤进展中HIF-2α、VEGF表达水平增高,并与骨巨细胞瘤骨破坏程度及局部复发呈正相关。
Objective To investigate the expression of hypoximinducible factor (HIF) and vascular endothelial growth factor (VEGF) related with the bone destruction in giant cell tumor of bone (GCTB). Methods Between June 2006 and July 2009, 18 patients with GCTB were reviewed in our department. The radiographs of the affected bone from each patient were studied and the extent of bone destruction was graded on the basis of Campanaeei's staging system for GCTB. Immunohistochemistry (IHC) was used to study the expression of HIF-α, HIF-2α and VEGF. Results Five patients of stage Ⅰ, 4 of stage Ⅱ, 5 of stage Ⅲ lesions, and 4 of recurrent lesions were observed. Immunohistochemical staining slices of tissue samples showed that 7 cases (7/18) were HIF-1α positive, all specimens were HIF-2α and VEGF positive. There was no significant difference in HIF-1α expression levels between the phases of the case group (P〈0. 05). HIF 2a, VEGF expression levels in stage Ⅱ, Ⅲ and recurrence group were significantly higher than that in stage Ⅰ group (P〈0.05). Conclusions The expression levels of HIF-2α and VEGF in advanced GCTB are significantly higher. There is a positive correlation with the extent of bone destruction and local recurrence.
