目的:研究MR IDEAL-QUANT及ESWAN成像技术在评估肝纤维化患者肝内铁质沉积中的应用价值。方法:选取44例血清铁指标异常的肝纤维化患者(血清铁异常组)与12例血清铁指标正常的肝纤维化患者(对照组),行MR IDEAL-QUANT、ESWAN扫描,测量R2*值,对其与血清学肝纤维化相关指标进行相关性研究。结果:血清铁异常组与对照组间的MR IDEAL-QUANT、ESWAN成像R2*值差异有统计学意义(P〈0.05)。血清铁异常组患者的R2*值(IDEAL-QUANT、ESWAN)与其血清铁蛋白水平间有中度正相关性(r=0.644、0.634、0.727、0.682,P〈0.05),其中部分R2*值则与其血清转铁蛋白水平间有轻度负相关性(r=-0.310、-0.400、-0.417,P〈0.05),余血清学指标与R2*值间无统计学相关性。结论:MR IDEAL-QUANT及ESWAN成像技术可用于观察肝纤维化患者肝内的铁质分布,并能定量评估肝内铁质沉积程度。
Objective: To study the value of IDEAL-QUANT and ESWAN imaging technique for evaluating iron deposition in liver fibrosis. Methods: MR IDEAL-QUANT and ESWAN scanning were performed in 44 liver fibrosis patients with abnormal serum iron indices (serum iron abnormal group) and 12 liver fibrosis patients with normal serum iron indices (control group). R2* value was measured and its correlation with liver fibrosis related serum indices was analyzed. Results: Statistically significant differences in R2* values (IDEAL-QUANT, ESWAN) were found between serum iron abnormal group and control group (P〈0.05). In serum iron abnormal group, moderate positive correlation was found between R2* values and ferritin(0.644,0.634, 0.727, 0.682), while mild negative correlation was seen between partial R2* values and transferrin (-0.310, -0.400, -0.417). Other serum indices had no correlation with R2* value (P〉0.05). Conclusions: MR IDEAL-QUANT and ESWAN imaging could be used for monitoring and evaluating the degree of iron deposition in liver fibrosis.
