目的：观察针刺三阴交、合谷联合硬膜外阻滞对足月分娩孕妇的镇痛疗效。方法：选择本院要求行分娩镇痛的初产妇48例．随机分为联合组和对照组，每组24例。联合组给予针刺三阴交、合谷配合硬模外阻滞镇痛，对照组仅给予硬膜外阻滞镇痛。在镇痛前（T1）、硬膜外阻滞镇痛后30min（T2）、硬膜外阻滞镇痛后1h（33）和宫口全开（T4）4个时间点分别观察孕妇宫腔压力、宫缩间歇时间、宫缩持续时间及VAS评分等4项指标，并详细记录孕妇分娩过程中硬膜外镇痛用药量、催产素用量、产后出血量、分娩方式、不良反应、新生儿Apgar评分及新生儿脐静脉血气。结果：两组VAS评分、子宫收缩情况及新生儿Apgar评分（5rain）等比较差异无统计学意义（P〉0．05）；两组孕妇及新生儿情况均良好，无不良反应出现；联合组硬膜外用药量、催产素明显少于对照组（P均〈0．05）；联合组新生儿Apgar评分（1min）、脐静脉血气等均明显优于对照组（P均〈0．05）。结论：针刺三阴交、合谷联合硬膜外阻滞镇痛在孕妇分娩中可有效减少镇静镇痛药物及催产素的使用量，明显改善新生儿一般情况，安全性较佳。

英文摘要：

Objective：To observe the analgesic effects of puncturing Sanyinjiao（SP6） and Hegu acupoints （LI4） combined with epidural block on the pregnant of term delivery. Methods ：48 pregnant women who demands analgesic delivery were randomized in- to combination group and control group, with 24 cases in each group. Combination group were given the combined treatment of puncturing Sanyinjiao （SP6） and Hegu acupoints （LI4） , and epidural block, while control group were merely given epidural block treatment. Four test times were set as before the analgesia （T1） ,30min after the epidural block （T2） , 1 hour after the epi- dural block （T3） , and full opening of the cervix （T4）. The uterine cavity pressure,intermittent time of the uterine contractions, duration of tile uterine contractions and VAS scores were observed at these four different test time. And the dosage of the epidural block and oxytocin,amount of postpartum bleeding, delivery methods,adverse reaction,neonatal Apgar score and umbilical venous blood gases were specifically recorded during the delivery. Results：The differences of the VAS scores, uterine contractions and neonatal Apgar scores （ 5 min ） between the two groups had no statistical significance （ P 〉 0.05 ）. The conditions of the pregnant and the neonatal were good in both groups anti there was no adverse reaction. The dosages of drugs used in epidural block and ox- ytocin in combination group were obviously lower than those in control group （P 〈 0, 05 ）. The neonatal Apgar scores （ 1 min） and umbilical venous blond gases in combination group were significantly better than these in control group （ P 〈 0.05 ）. Conclusion ： The combination of puncturing Sanyinjiao （SP6） and Hegu acupoints （LI4） anti epidural block can effectively reduce the use of analgesics and oxytocin on the pregnant of term delivery,and improve the general condition of the neonatal with good safety.
