股骨头坏死后骨盆倾斜角度对人工髋关节置换后功能的影响

目的分析股骨头坏死后骨盆倾斜角度对人工髋关节置换后功能的影响。方法选取我院于2015年8月~2016年7月期间收治的75例患侧盆下型骨盆倾斜股骨头坏死患者为研究对象,所有患者均接受人工髋关节置换术治疗。根据骨盆倾斜角度分为3型,即Ⅰ型(〈30)、Ⅱ型(3°~6°)、Ⅲ型(〉6°),分别为33例、32例及10例,比较三组患者髋臼前倾角与外展角.并于术后6个月评价患者髋关节功能恢复情况。结果Ⅰ型、Ⅱ型、Ⅲ型骨盆倾斜角度患者髋臼外展角分别为(44.75±1.87)°、(40.4±1.92)°、(37.34±1.83)°,三组相比,差异具有统计学意义(P〈0.05);术前、术后6个月I型骨盆倾斜角度患者髋关节Harris评分均明显高于Ⅱ型、Ⅲ型骨盆倾斜角度患者,且Ⅱ型骨盆倾斜角度患者髋关节Harris评分均明显高于Ⅲ型骨盆倾斜角度患者,差异具有统计学意义(P〈0.05)。结论股骨头坏死后骨盆倾斜角度对人工髋关节置换后髋关节功能具有一定影响,骨盆倾斜角度越大,患者髋关节功能越差。

英文摘要:

Objective To analyze the effect of pelvic tilt angle after femoral head necrosis on the function after artificial hip replacement. Methods A total of 75 patients with ipsilateral infrapelvic type femoral head necrosis in our hospital from August 2015 to July 2016 were enrolled. All patients received artificial hip replacement. And they were divided into 3 types ingcluding type I (〈3°), type 11 (3-6°) and type Ⅲ (〉6°) ,with 33 cases, 32 cases and 10 cases in each type, respectively. The acetabular anteversion angle and abduction angle in the three groups of patients were compared, and the hip function recovery of patients was evaluated at 6 months after surgery. Results The acetabular abduction angles were (44.75±1.87)°, (40.48±1.92)°and (37.34±1.83)°respectively in the patients with type I, II and type 111 pelvic tilt angles. The difference was statistically significant between the three groups (P〈0.05). Harris score of hip joint in patients with type I pelvic tilt angle was significantly higher than that of patients with type ]1 and type III pelvic tilt angles before surgery and at 6 months after surgery. The Harris score in patients with type Ⅱ pelvic tilt angle was significantly higher than that of patients with type Ⅲ pelvic tilt angle, and the difference was statistically signifieant(P〈0.05). Conclusion The tilt angle after femoral head necrosis has a certain effect on hip function after artificial hip replacement. The larger the angle of pelvic tilt is, the worse the hip function is.
