目的探讨痛风合并高血压患者同一高血压分级情况下服用不同种类降压药物对其代谢指标的影响。方法选取未行降压治疗或规律服用单一种类降压药物8周以上的痛风合并高血压患者902例，按照血压水平将其分为3组；比较3组高血压患者临床指标的差异及同一高血压等级患者在分别服用利尿剂、B受体阻滞剂、钙通道阻滞剂（CCB）、血管紧张素转化酶抑制剂（ACEI）及血管紧张素Ⅱ受体阻滞剂（ARB）进行降压治疗后的临床指标变化。结果随着血压水平的增高，受试者体质量指数（BMI）、血糖（PG）、甘油三酯（TG）、总胆固醇（TC）、尿素氮（BUN）、肌酐（Cr）、尿酸（SUA）均呈上升趋势；1级高血压患者中，利尿剂组TG、Cr显著高于未用药组，CCB、ACEI及ARB组TG、BUN、Cr、SUA水平显著低于利尿剂组，ARB组尿酸排泄分数（FEUA）较其他各组显著升高（P均〈0．05）；2级高血压患者中，CCB、ACEI、ARB组与利尿剂组及B受体阻滞剂组相比SUA降低，ACEI及ARB组较其他各组Cr降低，ARB组较其他各组FEUA升高（P均〈0．05）；3级高血压患者中ACEI及ARB组TG、SUA、Cr低于利尿剂组，ARB组FEUA显著高于其他各组（P均〈0．05）。结论痛风合并高血压患者应根据自身情况正确选择降压药物。

Objective To investigate the influence of different kinds of antihypertensive drugs on metabolism index of gout patients with hypertension under the same hypertension grading. Methods A total of 902 gout patients with hyperten- sion who were not treated or only accepted single variety of antihypertensive drug regularly for 8 weeks were enrolled. All of the patients were divided into three groups according to blood pressure. The biochemical data of the different three groups and the data of the same hypertension grading group after being taken diuretic, β-blocker, calcium channel blocker（ CCB）, angiotensin converting enzyme inhibitor（ACEI） or angiotensin receptor blocker （ARB） were compared. Results With the elevation of blood pressure, body mass index （ BMI）, plasma glucose （ PG）, triglyceride （TG）, cholesterol （ TC）, blood u- rea nitrogen（BUN）, creatinine （Cr）, serum uric acid （SUA） all showed an increasing tendency. Data of patients with grade 1 hypertension ： TG and Cr were much higher in diutetic group than in non-medication group. TG, BUN, Cr and SUA were much lower in CCB, ACEI and ARB group than in diuretic group, but FEUA was much higher（ all P 〈 0.05）. Data of patients with grade 2 hypertension ： SUA was much lower in CCB, ACEI, ARB group than in diutetic and β- blocker group. Cr was lower in ACEI and ARB group. FEUA was much higer in ARB group （all P 〈 0.05）. Data of patients with grade 3 hypertension ： TG, SUA were much lower in ACEI and ARB group than in duretic group, and CR was lower than in duretic and β- blocker group. While SUA in ARB group was much lower than in other groups （ all P 〈 0.05 ）. ConclusionGout patients with hypertension should be given antihypertensive drugs according to their self-condition correctly.
