目的探讨北京市不同性别成年人群代谢综合征(MetS)的发生与淋巴细胞数量的关联性。方法回顾性分析2007年1月1日~2012年12月31日在北京电力医院和北京小汤山医院进行健康查体的7489例体检者的体检资料,每例研究对象在研究期间至少进行3次及以上体检。从每次体检时的血常规检查结果中获取淋巴细胞数量资料,将不同性别体检者按照淋巴细胞数量四分位数分为Q1(≤25%)、Q2(〉25%-50%)、Q3(〉50%-75%)、Q4(〉75%)四个水平。根据末次体检结果筛选MetS患者,运用广义估计方程(GEE),分析MetS与淋巴细胞数量的关系。结果追踪体检者5年间,共发生MetS 378例,其中男203例、女175例,发病密度为12.67/1000人年。男性人群中,与Q1相比,淋巴细胞数量为Q4者的BMI升高(RR=1.4938,95%CI为1.0908~2.0457),Q2、Q3、Q4者的TG升高(RR分别为1.2084、1.4425、1.7077,95%CI分别为1.0323~1.4146、1.2103—1.7192、1.4065~2.0735)。女性人群中,与Q1相比,淋巴细胞数量为Q4者的MetS发病密度升高(RR=2.2008,95%CI为1.2219~3.9640)。结论北京市成年人群中,男性淋巴细胞数量增加与BMI升高、TG升高有关,女性淋巴细胞数量增加与MetS的发生有关。
Objective To explore the association between the count of lymphocyte and the occurrence of metabolic syndrome (Mets) in a cohort study in male and female Beijing adults specifically in gender. Methods Clinical data of 7 489 cases who took at least 3 health examination from 1st January 2007 to 3hh December 2012 were retrospectively analyzed in Beijing Electric Power Hospital and Beijing Xiaotangshan Hospital. The count of lymphocyte of research objects was obtained from the blood routine examination. And the lymphoeytes were divided into 4 levels according to the quartile (Q1≤25%, Q2:25 %-50%, Q3:50%-75 %, Q4 〉 75% ) in gender. Metabolic syndrome was diagnosed by the last data of health examination. Generalized estimating equation model was used to assess the relationship between metabolic syndrome and the lymphocyte. Results During the 5 years' follow-up, 378 cases appeared MetS, including 203 males and 175 females. And the incidence density of Mets was 12.67 per 1000 person-years. Compared with Q1, the males' BMI increased in Q4 (RR = 2. 200 8, 95% CI: 1. 090 8-2. 045 7 ), and the males' TG increased in Q1, Q2 and Q3 (RR = 1. 208 4, 1. 442 5,1. 707 7,95% CI: 1. 032 3-1. 414 6,1. 210 3-1. 7192,1. 406 5-2. 073 5). Compared with Q1, the females whose lymphoeytes were Q4 increased the risk of MetS (RR = 2. 200 8,95% CI: 1. 221 9-3. 964 0). Conclusions The increase of males' lymphoeytes may associate with the increase of BMI and TG, and the increase of females' lymphoeytes may associate with the occurrence of MetS in Beijing adults.
