Force CT Turbo Flash扫描模式联合迭代重建算法降低辐射剂量:主动脉CTA的比较研究

目的:利用Force CT的Turbo Flash模式行主动脉全程扫描成像,对照评价其图像质量,旨在评价该模式行主动脉全程扫描联合迭代重建算法降低辐射剂量的可行性.方法:选取内蒙古包头市中心医院2015-08/2016-12行主动脉全程CTA扫描的患者60例,将其分为实验组用Turbo Flash模式扫描(A组,n=30),使用迭代重建算法(ADMIRE)重建图像、对照组用Body Angio模式扫描(B组,n=30),使用滤波反投影算法(FBP)重建图像,并用客观和主观两种方式评价其CT图像,前者包括CNR、SNR及图像噪声,而后者则由两位CT影像诊断副主任医师采用双盲法分别评价.结果:Turbo Flash模式联合迭代重建算法图像的主观评分明显高于Body Angio扫描组,差异具有统计学意义(P〈0.05),而两组图像质量的客观评价比较,差异无统计学意义(P〉0.05).Turbo Flash模式组的扫描时间(1.0±0.2)s,相对于Body Angio模式(4.6±1.4)s明显缩短,差异具有统计些意义(P〈0.05).有效辐射剂量Turbo Flash模式组为(3.7±1.2)mSv,较Body Angio模式组的(6.8±2.8)mSv明显减少,差异具有统计学意义(P〈0.05).结论:Force CT Turbo Flash模式主动脉CTA扫描联合迭代重建算法的图像质量可满足对于主动脉病变的诊断需求,具有扫描时间短、辐射剂量低、运动伪影少等优势,可成为主动脉CTA优先选择的扫描模式推广应用.

英文摘要:

AIM: To investigate the feasibility,image quality and radiation dose for tube-flash protocol of force-CT with ADMIRE of the whole aorta. METHODS: Each group of 30 patients underwent CTA either on force-CT device with tube-flash protocol with ADMIRE( group A) or Body Angio protocol with FBP( group B). Evaluation of its image quality and radiation dose was evaluated objectively and subjectively. Objective evaluation indices included CNR,noise,SNR. Subject image quality of the aorta was independently assessed by two associate chief diagnosticians of CT imaging in a double-blind manner. Scanner-reported dose-length product values were used to estimate effective dose values. Radiation dose evaluation indices included CTDIvol,DLP and ED. RESULTS: The subjective scoring of image quality in the high-pitch examination protocol was obviously higher than that of group B,and the difference was statistically significant(P〈0.05). There was no statistically significant difference on objective evaluation of image quality between the two groups(P〈0.05).The imaging time of group A was( 1.0±0.2) s,which was significantly shorter than that of group B( 4.6±1.4) s,with a statistically significant difference(P〈0.05). Compared with group B( 6.8 ±2.8) mS v,ED of group A( 3.7 ± 1.2) mSv was significantly decreased,and the difference was statistically significant(P〈0.05). CONCLUSION: CT angiography of the whole aorta using tube-flash protocol of force-CT with ADMIRE can meet the need of patients with aortic diseases with shorter scan time,lower radiation dose and less motion artifacts,which has the potential to become a preferred application of aorta CTA.
