目的探讨慢性肝囊型包虫持续感染患者外周血单核细胞的亚群组成及其功能状态。方法囊型包虫病患者(CE组)和健康者(NC组)各15例,采集外周血单核细胞(PBMC)后分成两部分,一部分应用流式细胞术分析其表面CD14、CD16的阳性表达率;另一部分分为未干预组和重组抗原B(rAgB)干预组,干预后ELISA法检测上清中IFN-γ、IL-12p70、IL-10、IL-4的浓度。结果 PBMC占外周血白细胞的比例在CE组、NC组分别为(13.42±3.90)%和(6.23±1.39)%,差异有统计学意义(t=6.17,P〈0.05);CD14阳性表达率分别为(19.90±6.96)%和(45.61±14.25)%,差异有统计学意义(t=6.28,P〈0.05);CD16阳性表达率分别为(22.75±8.99)%和(22.83±6.77)%,差异无统计学意义(t=0.03,P〉0.05)。除NC组rAgB干预后的IL-4与未干预对照相比差异无统计学意义外,来源于NC组、CE组rAgB干预后的4种细胞因子浓度与未干预对照相比差异均有统计学意义(P均〈0.05)。CE组与NC组相比,rAgB干预下的IFN-γ、IL-12p70及IL-10的浓度差异均有统计学意义(t值分别为2.580,2.745,2.213,P值均〈0.05),IL-4浓度差异无统计学意义(t=0.722,P〉0.05)。结论慢性CE患者PBMC的CD14低表达致使单核细胞未能达到成熟状态,影响其分化能力,同时联合细粒棘球绦虫主要抗原成分的作用使机体倾向于Th2型反应,是囊型包虫病慢性感染引起机体免疫低下的重要因素。
Objective To investigate the subsets and function of peripheral blood mononuclear cells(PBMCs) from patients with hepatic cystic echinococcosis(CE).Methods Fifteen patients with hepatic cystic echinococcosis(CE group) who were treated at the First Affiliated Hospital of Xinjiang Medical University and 15 healthy people(NC group) were enrolled in this study.PBMCs were isolated from the CE and NC groups by density gradient separation and divided into 2 samples.The first sample was analyzed by flow cytometry for expression of CD14 and CD16 while the second sample was cultured in vitro.PBMCs from the CE and NC groups that were subjected to in vitro culturing were further divided into 2 groups;in 1,PBMCs received no intervention group while in the other PBMCs were subjected to intervention with recombinant antigen B(rAgB)(final concentration: 15μg/ml).After being subjected to intervention for 24 hours,cell supernatants were collected and the concentration of IFN-γ,IL-12p70,IL-10,and IL-4 was determined with ELISA.Results The ratio of PBMCs in the CE group and NC group was(13.42±3.90)% and(6.23±1.39)%,respectively.The ratio of PBMCs in the two groups differed(t=6.17,P0.05).The rate of expression of CD14 was(19.90±6.96) % and(45.61±14.25) %,respectively.The level of expression differed significantly between the CE group and NC group(t=6.28,P0.05).The rate of expression of CD16 was(22.75±8.99) % and(22.83±6.77) %,respectively.There was no difference in the level of expression(t=0.03,P0.05).When PBMCs subjected to rAgB intervention were compared to PBMCs subjected to no intervention,there were significant differences in the 4 cytokines for the CE and NC groups(all P values 0.05).In the NC group,there was no difference in the concentration of IL-4 for PBMCs subjected to rAgB intervention and PBMCs subjected to no intervention.In the CE group,there were significant differences in the concentration of IFN-γ,IL-12p70,and IL-10(t= 2.580,2.745,2.213,re
