目的 研究银川市某医院患者住院费用分布情况,探寻患者住院费用的影响因素,从而为减轻患者经济负担提供合理可行的建议。方法 采用描述性分析,从医院HIS系统中收集2014—2016年住院患者病案首页信息,分析住院患者基本情况,采用单因素和多元线性回归分析住院费用的影响因素。运用SPSS 20.0软件进行统计分析。结果 本次研究共纳入10 156例住院患者,2014年其住院费用中位数为9 055.90元,2015年为9 990.21元,2016年为9 099.52元;一般诊疗服务费、药费和材料费是患者住院费用的主要构成;60~70岁患者的次均住院费用最高;住院天数在26天以上的患者住院费用占比最高;影响患者住院费用的主要因素是性别、年龄、住院天数和转归情况（P〈0.05）。建议控制药费所占比例;减少不合理住院天数;病人合理分流就诊,强化社区卫生服务功能;减少保护性医疗行为。

OBJECTIVE To study the distribution of hospitalization expenses in a hospital of Yinehuan, explore the patient hospitaliza- tion expenses and influencing factors, provide reasonable and feasible suggestions to shorten hospital unreasonable length of hospital stay, control unreasonable medical expenses of rapid growth and reduce the economic burden of the patients. METHODS Describe the distribution of hospitalization costs, univariate analysis of the possible influencing factors of hospitalization costs; multivariate lin- ear regression was used to analyze the effect of influencing factors on hospitalization costs. RESULTS A total of 10 156 cases of dia- betes were included. The median of total median costs and daily 9 055.90 yuan in 2014. the fee for 2015 was 9 990.21yuan and the fee for 2016 is 9 099.52 yuan; the average cost of hospitalization was highest in patients aged 60 - 70 years; the hospitalization costs of patients with hospital days of 26 days were the highest; the factors including gender, age, length of hospital stay and treatment out- come were proved to be significantly correlated to total medical costs of diabetes impatiens. SUGGESTIONS It needed to control drug proportion; to reduce the irrational hospitalization days; rational diversion of patients to visit a doctor, strengthen the function of com- munity health service and reduce the protection of medical behavior.
