目的检测ⅢA期肺鳞癌和腺癌患者术后癌及癌旁组织中的白介素17(IL-17)、Foxp3的表达情况,分析IL-17与Foxp3的相关性及与患者预后的关系,探讨肺鳞癌和腺癌组织中辅助性17(Thl17)与Foxp3^+调节性T(Foxp3^+Treg)细胞浸润的临床意义。方法采用免疫组织化学法检测62例术前未行化疗或放疗的汉族肺鳞癌和腺癌患者的手术切除标本癌组织、癌旁组织中IL-17和Foxp3表达。结果IL-17在肺鳞癌和腺癌组织中阳性表达53例(85.5%),相应癌旁组织中阳性表达35例(56.5%),癌组织和癌旁组织的IL-17阳性表达率比较,差异有统计学意义(X^2=6.779,P=0.009)。Foxp3在肺鳞癌和腺癌组织中阳性表达48例(77.4%),相应癌旁组织中阳性表达34例(54.8%),癌组织和癌旁组织的Fox03阳性表达率比较.差异有统计晕意义X^2=5.038.P=0.025、肺癌组织申IL-17表达与Foxp3表达呈正相关(r=0.309,P=0.010)。IL-17阳性表达组和阴性表达组的生存率比较,差异有统计学意义(X^2=9.386,P=0.002);Foxp3阳性表达组和阴性表达组的生存率比较,差异有统计学意义(X^2=14.069,P=0.000)。Cox回归模型分析显示IL-17阳性表达(P=0.046,OR=2.359)及Foxp3阳性表达(P=0.003,OR=3.180)是影响肺鳞癌和腺癌患者预后的独立危险因素;组织学类型(P=0.162,OR=1.518)及年龄(P=0.292,OR=1.019)不是影响预后的危险因素。结论Thl7细胞特异性分泌IL-17,Treg细胞最可靠的特征性标记是Foxp3,肺鳞癌和腺癌患者肿瘤组织中IL-17和Foxp3较自身癌旁组织阳性表达率高,癌组织中IL-17与Foxp3表达呈正相关。IL-17和Foxp3阳性表达均是影响预后的独立危险因素;IL-17及Foxp3是影响肺鳞癌和腺癌发展和预后的重要免疫学指标。提示Th17和Foxp3^+Treg细胞与肺鳞癌、腺癌的发生、发展有关,但尚需大规模的?
Objective To investigate the expression of IL - 17 and Foxp3 in m A stage lung squamouscellcarcinoma and adenocarcinoma and to analyze the expression and correlation of IL - 17 and Foxp3 and their relationship with prognosis in order to evaluate the clinical values of Thl7 and Foxp3 ^+ Treg ceils in cellular infiltration. Methods The expressions of IL - 17 and Foxp3 in surgical resection specimens of lung squamouscellcarcinoma and adenocarcinoma tissues and surrounding tissues of 62 Han nationality patients who had neither radiotherapy nor chemotherapy before operation were detected by Immunohistochemistry method. Statistical study was performed with Software SPSS 15.0 for Windows. Results The positive expression of IL - 17 in lung squamouscellcarcinoma and adenocarcinoma was found in 53 cases (85.5 % ) , while its positive expression in surrounding tissues was found in 35 cases (56. 5% ), the difference was statistically significant (X^2 =6. 779, P =0. 009) . The positive expression of Foxp3 in lung squamouscellcarcinoma and adenocarcinoma was found in 48 cases (77.4%), while its positive expression in surrounding tissues was found in 34 cases (54. 8% ), the difference was statistically significant (X^2 = 5. 038, P = O. 025) . Theexpression of IL - 17 was positively correlated with the expression of Foxp3 in lung carcinoma tissues ( r = 0. 309, P = 0. 010 ).
