【目的】观察以解毒软肝饮为主方辨证加减治疗门静脉高压症的临床疗效。【方法】收集60例门静脉高压症患者，随机分为治疗组40例和对照组20例。治疗组以解毒软肝饮为基础方辨证加减治疗。对照组给予口服谷胱甘肽含片、心得安、复方丹参片治疗。2组均以3个月为1个疗程，分别于治疗前和治疗1、2、3个月后观察患者临床见证、不良反应、理化检查、门静脉血流动力学以及肝纤维化等各项指标的变化。【结果】2组治疗后的见证积分、理化检查、门静脉血流动力学、肝纤维化等多项指标均有显著改善，与治疗前比较差异有统计学意义（P〈0．05或P〈0．01）。治疗后治疗组多项指标改善优于对照组，2组比较差异有统计学意义（P〈0．05或P〈0．01），表明治疗组对门静脉高压症患者临床体征的改善，肝功能生化指标的恢复，胃黏膜征的修复，食营、胃底静脉曲张以及肝脾肿大的改善都有较好疗效，在缩小门静脉、脾静脉血管内径，平稳维持门静脉、脾静脉的血流速度和血流量等血流动力学变化，以及改善肝纤维化标志物指标方面均优于对照组。【结论】以解毒软肝饮为基础方辨证加减治疗门静脉高压症疗效明确，安全可靠，无明显不良反应和毒副作用。

英文摘要：

Objective To observe the clinical efficiency for portal hypertension treated mainly with Jiedu Ruangan Decoction （JRD） based on syndrome differentiation. Methods Sixty portal hypertension patients were randomized into treatment group （N = 40 ） and control group （N = 20 ） . The treatment group was treated with modified JRD based on syndrome differentiation, and the control group was given oral use of Glutathione Tablets, Xinde＇an Tablets and Compound Danshen Tablets. Three months constituted one treatment course. Clinical symptoms and signs, adverse effect, biochemical parameters, portal venous hemodynamics, and hepatic fibrosis parameters were observed before treatment, and one, 2 and 3 month （s） after treatment. Results After treatment, the scores of clinical symptoms and signs, biochemical parameters, portal venous hemodynamics, and hepatic fibrosis parameters were improved in both groups （ P 〈 0. 05 or P 〈 0. 01 compared with those before treatment）, and the differences between the two groups after treatment were significant （P 〈 0. 05 or P 〈 0. 01 ）, which indicated that the treatment group had better effect on relieving symptoms and signs, improving hepatic biochemical parameters, promoting the recovery of gastric mucosa, relieving esophageal and gastric fundus phlebeurysma as well as splenohepatomegalia, reducing inside diameter of portal veins and spleen veins, stabilizing blood flow rate and hemodynamics of portal veins and spleen veins, and improving hepatic fibrosis markers. Conclusion The treatment of portal hypertension mainly with JRD based on syndrome differentiation is effective and safe, without adverse reaction or toxic and side effects.
