目的探讨不同糖耐量人群血清中含BED锌指蛋白3(ZBED3)水平的变化及其与胰岛素抵抗的关系。方法以2型糖尿病(T2DM,n=109)、糖耐量正常(NGT,n=113)、糖耐量受损(IGT,n=89)者为研究对象。采用酶联免疫法测定患者血清ZBED3水平,分析其与人体学指标如血压、腰臀比、体重指数(BMI)及其他相关代谢指标的关系,并分析高胰岛素、高血糖对ZBED3的影响。结果T2DM组血清ZBED3水平明显高于IGT组和NGT组[(142.1±31.0对122.2±25.4和96.0±19.7)ng/L,均P〈0.01];女性人群中血清ZBED3水平明显高于男性[(122.0±34.6对114.7±26.6)ng/L,P〈0.05];超重组血清ZBED3水平高于非超重组[(131.1±31.6对88.6±17.1)ng/L,P〈0.01)];血清ZBED3水平与BMI、腰臀比、体脂百分比(BF%)、收缩压、舒张压、甘油三酯(TG)、空腹血糖、空腹胰岛素、HbA1c、稳态模型评估的胰岛素抵抗指数(HOMA.IR)呈明显正相关(均P〈0.01),与高密度脂蛋白胆固醇呈负相关(P〈0.01),多元逐步回归分析显示腰臀比、BF%、舒张压、TG、HbA1c、HOMA.IR是影响血清ZBED3的独立相关因素;糖耐量正常者口服75g葡萄糖30min后血清ZBED3从基线(96.0±19.7)ng/L快速上升到(169.0±39.1)ng/L(P〈0.01),而后逐渐下降,2h时为(93.5±29.1)ng/L。结论血清ZBED3在T2DM、IGT患者及超重人群中均明显升高,其可能与胰岛素抵抗发生发展相关,并受血糖和胰岛素影响。
Objective To evaluate the plasma zinc-finger BED domain-containing 3 ( ZBED3 ) levels in subjects with different status of glucose tolerance, and to explore the relationship between ZBED3 and insulin resistance. Methods There were 109 patients with newly diagnosed type 2 diabetes mellitus (T2DM), 89 individuals with impaired glucose tolerance ( IGT), 113 sex-, age-, and body mass index (BMI)-matched subjects with normal glucose tolerance (NGT) included in this study. Plasma ZBED3 level was determined by ELISA. The relationships of plasma ZBED3 with metabolic parameters, hyperglycemia, and hyperinsulinemia were analyzed. Results Plasma ZBED3 level was higher in T2DM group than those in IGT and NGT groups [ ( 142.1 ± 31.0 vs 122.2 ± 25.4 and 96.0 ± 19.7 ) ng/L, both P〈0.01 ]. Women had higher plasma ZBED3 level than men did [ ( 122.0± 34.6 vs 114.7 ± 26.6 ) ng/L, P〈0.05 ]. There was higher ZBED3 level in overweight patients compared with lean individuals [ ( 131.1 ± 31.6 vs 88.6 ± 17.1 ) ng/L, P〈 0.01 ) ]. Plasma ZBED3 level was positively correlated with BMI, waist-hip rate (WHR), body fat% (BF%), systolic blood pressure, diastolic blood pressure (DBP), triglyeeride (TG), fasting blood glucose, fasting insulin, HbA1c, and homeostasis model assessment insulin resistance index ( HOMA-IR ), while inversely correlated with high density lipoprotein-cholesterol ( P 〈 0. 01 ). Moreover,logistic analysis showed that WHR, BF% , DBP, TG, HbA1c, and HOMA-IR were independently related to plasma ZBED3 level. Circulating ZBED3 level in normal subjects was rapidly elevated from ( 96.0 ± 19.7 ) ng/L at 0 min to ( 169.0 ± 39. 1 ) ng/L at 30 min during oral glucose tolerant test ( P〈0.01 ) , then gradually decreased to ( 93.5 ±29.1 ) ng/L at 2 h.
