目的:研究首发抑郁症患者血清炎性因子水平变化对海马区引流静脉-侧脑室下静脉形态的影响。方法:采集20例首发抑郁症患者及20例健康志愿者外周静脉血,检测血清中IL-6、IL-2及IL-10水平,之后采用磁敏感序列(SWI序列)进行磁共振扫描,检测双侧海马区引流静脉的最大直径、长度、分支数目,并将外周血清各炎性因子指标水平与引流静脉形态变化进行相关性分析。结果:与正常组对比,首发抑郁症患者的血清IL-6、IL-2水平出现有统计学意义的升高,IL-10水平变化不明显;首发抑郁组患者左侧脑室下静脉的最大直径出现有统计学意义的变窄,而长度及分支出现增高,但无统计学意义;右侧脑室下静脉的最大直径出现有统计学意义的变窄,长度及分支未见明显变化。左侧脑室下静脉的最大直径与血清IL-6及IL-2水平亦成明显负相关,右侧脑室下静脉的最大直径与血清IL-6及IL-2水平亦成明显负相关。结论:首发抑郁症患者处于一种慢性炎性状态,这些炎性因子水平的变化能够通过影响脑区引流静脉而干扰神经细胞的正常代谢,成为首发抑郁症发生发展的一个潜在机制。
Objective To explore the veins changes of the bilateral hippocampus effected by inflammatory cytokines levels changes about the first episode depression. Method Collecting venous blood of 20 first episode depression patients and 20 healthy persons,detecting the levels of IL-6、IL-2 and IL-10 in blood serum,then,using subsequence of SWI to scan their head,detecting maximum diameter、length and branch numbers of the bilateral hippocampus draining veins,finally,analyzing the correlation between them. Results Compared with the healthy group,the depressive group had the significantly higher levels of IL-6、IL-2 in blood serum,but no significant difference was found in the IL-10 level in blood serum; The maximum diameter of left inferior ventricular vein was reduced significantly,the length and branch numbers were increased,but not significantly; the maximum diameter of right inferior ventricular vein was reduced significantly,but the length and branch numbers were changed unobvious. The maximum diameter of left inferior ventricular vein was negatively correlated to the levels of IL-6、IL-2,the maximumdiameter of right inferior ventricular vein was negatively correlated to the levels of IL-6、IL-2 too. Conclusion The first episode depression patients is in chronic inflammation position,these changes of inflammatory cytokines levels can effect the nerve cell metabolism by draining veins,this way maybe one of the potential mechanisms in depression.
