目的综合评价结直肠癌患者术后手术部位感染的危险因素,为制定结直肠癌手术部位感染预防控制措施提供科学依据。方法计算机检索外文数据库(MEDLINE、EMBASE、Cochrane)和中文数据库(CNKI、万方、维普、CBM),并辅以手工检索、文献追溯等方法,收集2000年1月-2015年5月公开发表的关于结直肠癌手术部位感染危险因素的研究文献,经Newcastle-Ottawa Scale(NOS)标准质量评价后,对符合要求的文献进行定量(荟萃分析)和定性分析。结果共纳入12篇文献,累计结直肠癌手术病例1 1064例;荟萃分析结果显示,合并糖尿病、术前白蛋白低、Ⅲ/Ⅳ期癌症、腹腔镜为感染因素;定性分析结果显示,体质量指数(BMI)高、手术持续时间长将增加结直肠癌患者术后感染的风险。结论结直肠癌手术患者发生手术部位感染与多种因素有关,但腹腔镜手术为保护因素。
OBJECTIVE To comprehensively evaluate the risk factors for surgical site infections in colorectal cancer patients so as to provide scientific basis for development of control and prevention measures of surgical site infections in the colorectal cancer patients.METHODS The computers were used to retrieve the published literatures from the foreign databases such as MEDLINE,EMBASE,and Cochrane as well as the Chinese databases such as CNKI,Wanfang,VIP,and CBM,accompanied by the manual searching and literature tracing.The research literatures in respect of the risk factors for surgical site infections in the colorectal cancer patients that were published from Jan 2000 to May 2015 were collected,and the quantitative analysis(meta-analysis)and qualitative analysis were performed for the eligible literatures after the standard quality was evaluated with Newcastle-Ottawa Scale(NOS).RESULTS Totally 12 literatures were included in the study,involving 1 1064 colorectal cancer surgery patients.The results of the meta-analysis indicated that the risk factors for the infections included the complication of diabetic mellitus,low preoperative albumin level,stageⅢ/Ⅳ cancer,and laparoscopes.The results of the qualitative analysis showed that the high body mass index(BMI)and long operation duration may increase the risk of postoperative infections in the colorectal cancer patients.CONCLUSION There are a variety of risk factors for the surgical site infections in the colorectal cancer patients,however,the laparoscopic surgery is a protective factor.
