目的:建立家兔慢性心肌冬眠模型。方法:实验于2006-02/12在福建省冠心病研究所完成。实验分组:选择健康家兔30只,按随机数字表法分为模型组(n=18)和假手术组(n=12):模型组采用开胸不全结扎冠状动脉左前降支中段,关胸后继续喂养1个月,造成心肌慢性缺血,构建家兔慢性冬眠心肌模型。假手术组仅开胸不作冠状动脉左前降支中段结扎。实验评估:①采用冠状动脉造影观察冠状动脉左前降支中段狭窄程度:②超声心动图、多巴酚丁胺超声心动图负荷实验观察慢性缺血心肌室壁运动情况一③氯化三苯基四氮唑染色检测心肌坏死程度。④苏木精一伊红常规染色利用光学显微镜观察心肌组织结构变化=(9苦昧酸一天狼猩红胶原纤维染色观察心肌问质胶原纤维增生情况?结果:术后模型组家兔15只和假手术组12只进入结果分析:①狭窄程度:模型组冠状动脉左前降支中段直径平均狭窄率为(84.90±4.24)%,假手术组未见明显狭窄,②左心室整体收缩功能:模型组左室射血分数、短轴缩短率显著低于假手术组,差异有显著性意义(f=10.314,11.938,P〈0.01)。⑧心肌室壁运动情况:静息时模型组左室壁异常运动节段47个。多巴酚丁胺超声心动图负荷实验34个节段出现室壁运动障碍程度先加重而后改善的典型冬眠心肌的双相反应:④心肌坏死程度:氯化三苯基四氮唑染色显示模型组3只家兔出现小灶性透壁心肌坏死,其余12只家兔均未发现心肌坏死:假手术组12只家兔均未发现心肌坏死:⑧心肌组织结构变化:苏木精一伊红染色显示模型组心肌纤维肿胀,染色不均匀,横纹模糊,核周水肿;假手术组心肌纤维排列整齐、连接紧密。(9心肌问质胶原纤维增生情况:苦昧酸一天狼猩红胶原纤维染色显示模型组心肌问质、血管周围可见?
AIM: To establish the animal model of chronic hibernating myocardium (CHM) in rabbits. METHODS: The experiment was carried out in the Fujian Institute of Coronary Artery Disease from February to December 2006. Thirty healthy rabbits were randomly assigned into the following two groups by random digits table, namely model group (n =18) and sham operation group (n =12). Thoracotomy was conducted in middle left anterior descending (LAD) coronary artery without complete ligation in the model group, and then fed successively for 1 month to create a CHM model. Rabbits in the sham operation group only received thoracotomy without ligation. ①Coronary angiography was conducted to detect the stenosis of LAD. ②Echocardiography and Dobutamine Stress Echocardiography was performed to observe left ventricular segmental wall motion. ③TTC stain was conducted to detect myocardial necrosis. Hematoxylin-eosin (HE) staining examination was used to detect the histological changes of myocardium. Picrosidus-red staining was performed to observe the hyperplasy of interstitial collagen fiber in myocardium. RESULTS: Totally 15 rabbits of model group and 12 rabbits of sham operation group were involved in the result analysis. ①Stenosis level: The average stenosis rate of LAD was (84.90±4.24)% in model group, whereas no obvious stenosis was found in sham operation group. ②The whole systolic function of left ventricle: Left ventricular ejection fraction (LVEF) and shortening fraction in the model group were significantly lower than those in the sham operation group (t =10.314,11.938,P 〈 0.01 ). ③Myocardial wall motion: Forty-seven segments with abnormal motion were found in left ventricular wall. In Dobutamine Stress Echocardiography, thirty-four out of forty-seven segments showed a biphasic response, i.e. a deterioration during dobutamine infusion followed by a recovery after stopping dobutamine infusion. ④Myocardial necrosis: TTC stain showed that there were localized myocardia
