目的探讨肝功能对行钆塞酸二钠增强MR胆管成像的影响。方法将39例肝脏疾病患者按照Child-Pugh分级分为3组:A级组23例,B级组11例,C级组5例。均在注射钆塞酸二钠后50rain行胆管成像。用5分法评价胆总管、左肝管、右肝管、肝内二级胆管、胆囊管和胆囊的显像情况,测量胆总管的相对信号强度(RSI),记录相关实验室检查结果并计算终末期肝病模型评分(MELD)。用Kruskal.Wallis检验比较3组间各级胆管的显像评分情况,两两比较采用Bonferroni法;单因素方差分析比较3组间胆总管RSI;回归分析观察全部患者胆总管相对信号强度的影响因素。结果Child—PughA级组胆总管、左肝管、右肝管、肝内二级胆管、胆囊管和胆囊的显像评分中位数(下四分位数,上四分位数)分别为4(3,4)、3(2,d)、3(2,4)、1(1,2)、3(2,4)和3(2,4),B级组分别为3(0,3)、1(0,2)、1(0,2)、0(0,1)、2(0,2)和2(0,2),C级组分别为1(1,2)、1(1,1)、1(1,1)、0(0,0)、0(0,1)和0(0,1),差异均有统计学意义(x2值分别为17.881、18.707、23.328、13.016、20.170、19.938,P值均〈0.01);A、B级组,A、C级组评分差异也均有统计学意义(P值均〈0.01)。3组的胆总管相对信号强度依次为17.12±0.41、3.95±0.63和3.33±0.30,差异有统计学意义(F=235.843,P〈0.01);A、B级组,A、C级组间的差异有统计学意义(P值均〈0.01)。血清总胆红素及MELD评分是胆总管相对信号强度重要的预测因素(偏回归系数值分别为-0.834、-1.042,P值均〈0.01)。结论不同肝功能的Child-Pugh分级明显影响了钆塞酸二钠增强MR胆管成像及胆总管相对信号强度,胆总管相对信号强度能在一定程度上反映肝脏储备功能。
Objective To evaluate the effect of liver function on the biliary visualization of MR cholangiography (CE-MRC) using gadolinium ethoxybenzyl diethylenetriamine pentaacetie acid (Gd-EOB- DTPA). Methods Thirty-nine patients were divided into three groups based on Child-Pugh score (23 patients in group A, 11 in B, and 5 in C). CE-MRC was scanned 50 minutes after Gd-EOB-DTPA injection. Biliary visualization of CE-MRC at that time was qualitatively assessed by 5-point scale. The relative signal intensity ( RSI ) of the common bile duct (CBD) was quantitatively measured. The serum bilirubin level and the model of end-stage liver disease (MELD) score were recorded. Kruskal-Wallis test was used to compare the rating scores among three groups. Bonferroni method was used to further pairwise comparison. The one-way ANOVA was used to compare the RSI of CBD among three groups. And regression analysis was used to evaluate the relationship between RSI and the influenced factors. Results Visualization rating scores of the CBD, left hepatic duct (LHD), right hepatic duct (RHD), subbranches of intrahepatie bile ducts (SIBD) ,cystic duct (CD) and gallbladder (GB) were 4 (3,4) ,3(2,4) ,3(2,4) ,1(1,2) ,3(2, 4) ,3(2,4) of group A; 3(0,3) ,1(0,2) ,1(0,2) ,0(0,1) ,2(0,2) ,2(0,2) of group B; and 1 (1,2),1 ( 1,1 ), 1 ( 1,1 ), 0 ( 0,0 ), 0 ( 0,1 ), 0 ( 0,1 ) of group C. Statistically significant differences were found in visualization rating scores of the CBD, LHD, RHD, SIBD, CD and GB among the 3 groups ( X2 = 17. 881, 18. 707,23. 328, 13. 016,20. 170, 19. 938, P 〈 0. 01 ) . Statistically pairwise comparison differences were found between group A and B ,either between group A and C.
