目的探讨磁共振成像(MRI)扩散加权成像对乳腺良恶性病变的鉴别诊断价值。方法收集乳腺病变63例(恶性肿瘤41例,良性病变22例),进行扩散加权成像,采用单次激发回波平面成像技术取3个扩散敏感度值(b值),分别为0、500、1000s·mm^-2,分别测量不同b值下良恶性病变的表观弥散系数(ADC)值。结果随着b值的升高,乳腺病变ADC值减少。取b值为1000、0s·mm^-2时计算ADC值,良性病变组的为(1.257±0.204)×10^-3mm^2·s^-1,恶性病变组的为(0.796±0.147)×10^-3mm^2·s^-1(t=7.144,P〈0.001);取b值为500、0s·mm^-2时计算ADC值,良性病变组的为(1.371±0.224)×10^-3mm^2·s^-1,恶性病变组的为(0.872±0.104)×10^-3mm^2·s^-1(t=8.246,P〈0.001)。结论乳腺良恶性病变MRI扩散加权成像在不同b值下ADC值不同,通过测量ADC值可以对乳腺病变的良恶性进行鉴别诊断。
Objective To investigate the differential diagnostic significance of diffusion weighted magnetic resonance imaging (MRI) in distinguishing malignant from benign breast lesions. Methods Sixty-three patients with lesions (41 malignant lesions and 22 benign lesions) were enrolled in this study, and all the lesions were pathologically confirmed. The patients were scanned with 3 b values (0, 500, 1 000 s·mm^-2) in diffusion weighted imaging and the apparent diffusion coefficient (ADC) value was calculated. Results The ADC value of the lesions was decreased as b value increases. When b values were 1 000 and 0 s·mm^-2, the ADC values of benign and malignant lesions were (1. 257 ±0. 204) ×10^-3mm^2·s^-1 and (0.796±0. 147) ×10^-3mm^2·s^-1, respectively (t = 7. 144, P 〈0. 001 ). Conclusion When b values are different, the ADC values are different of MRI diffusion weighted imaging of benign and malignant breast diseases, the differentiation of malignant from benign breast lesions by ADC value is applicable.
