目的通过观察兔眼小梁切除术后,不同剂量C-ski基因重组质粒转染对眼压、滤过泡、滤过道炎症细胞、Ⅰ型和Ⅲ型胶原纤维、肌成纤维细胞的影响,了解C-ski基因对滤过道抗瘢痕化的作用。方法36只新西兰大白兔,随机分为4组,每组9只,分别对兔右眼行小梁切除术,于术毕辅以结膜下注射0.01mol/L的PBS溶液0.1ml(A组)、C-ski基因重组pcDNA3.0质粒100μg(B组)、300μg(C组)和500μg(D组)进行治疗,并于术后第1、3、5、7、14、21、30天观察滤过泡情况并测量双眼眼压,第7、14、30天分批处死家兔(每次每组3只),取右眼制作石蜡切片,分别进行HE、天狼星红、免疫组化染色,观察炎症细胞、Ⅰ型和Ⅲ型胶原纤维、肌成纤维细胞情况。结果A组与B组的两眼眼压差、滤过泡、Ⅰ型和Ⅲ型胶原纤维、炎症和肌成纤维细胞数量在各个时间点差异无统计学意义(P〉0.05)。D组双眼眼压差在第7~21天与A组比较,差异有统计学意义(P〈0.05),Ⅰ型胶原量在第14天后与A组间比较,差异有统计学意义(P〈0.05),C组各项测量指标在各个时间点与B、D组比较,差异均无统计学意义。结论将不同剂量的C-ski基因重组pcD-NA3.0质粒用于兔眼小梁切除术后抗瘢痕治疗,均未引起明显炎症反应,且随转染的C-ski基因剂量的逐渐增大其抑制I型胶原生成的作用越明显。500μg剂量的C-ski基因重组质粒能明显延长滤过泡生存时间,维持术后低眼压状态。
Objective To study the effect of C-ski gene on scar formation in filtering passageway after trabeculectomy by observing the influence of different amount of C-ski gene recombinant plasmid on intraocular pressure (IOP), filtering bleb, inflammatory cells, type I and type Ⅲ collagen fibers and myofibroblasts in filtering passageway. Methods Thirty-six rabbits were randomly divided into four groups (9 each). Trabeculectomy was performed in right eye, followed by topical application of 0.1ml of 0.01mol/L PBS solution (group A), 100μg C-ski gene recombinant plasmid pcDNA3.0 (group B), 300μg C-ski gene recombinant plasmid pcDNA3.0 (group C) or 500μg C-ski gene recombinant plasmid pcDNA3.0 (group D). Filtering bleb formation and IOP was evaluated on days 1, 3, 5, 7, 14, 21, and 30 after surgery. On 7th, 14th and 30th days the eyes were enucleated, the inflammatory cells, type I and Ⅲ collagen fibers and myofibroblasts in filtering passageway were assessed histopathologically and immunohistochemically. Results No significant difference was found between group A and group B at various time points on IOP, filtering bleb, inflammatory cells, type Ⅰ and Ⅲ collagen fibers and myofibroblasts in filtering passageway. The IOP was lower in group D than in group A during 7th day to 21st day, and the type Ⅰ collagen fiber was significantly reduced in filtering passageway in group D than in group A after 14th day (P0.05). No significant difference was observed between group C and group B or group C and group D at various time points in all the parameters mentioned above. Conclusion Trabeculectomy with topical application of C-ski gene recombinant plasmid may reduce the synthesis of type Ⅰcollagen fibers and inhibit cicatrix formation after operation, and 500μg C-ski gene recombinant plasmid may prolong the life of filtering blebs and lower IOP after trabeculectomy.
