目的采用局部一致性分析方法分析偏头痛患者的静息态功能磁共振成像影像,探讨偏头痛患者静息状态下自发神经活动特点及脑功能变化。方法选取31例偏头痛患者,以及性别、年龄和受教育年限相匹配的30例健康对照者。进行静息态功能磁共振扫描,数据的预处理和全脑的ReHo值采用DPARSF软件计算。两组全脑ReHo值的组间比较行t检验,采用rest软件计算,观察偏头痛患者组和健康对照组静息状态脑功能差异。结果与健康对照组相比,偏头痛患者多个情绪加工和认知控制相关脑区的自发神经活动异常,如额叶、颞叶和顶叶等,其中双侧前扣带回、双侧直回、右侧楔前叶、右侧后扣带回、右顶下小叶、右颞中回、右颞上回表现出功能一致性增强(P〈0.01);双侧辅助运动区、双侧旁中央小叶、右额内侧回、左背侧前扣带回、左边缘叶功能一致性减弱(P〈0.01)。结论静息状态下偏头痛患者存在多个相互作用的脑区构成的脑网络功能一致性异常。这些区域多与认知控制和情绪加工有关。
Objective To investigate the spontaneous neuronal activity (SNA) and brain function alteration in migraine patients during interictal period by regional homogeneity (ReHo) analysis with resting state functional magnetic resonance imaging (rs-fMRI). Methods Thirty-one patients with migraine during interictal period and 30 healthy controls, matched in age, sex and education level, were enrolled and scanned with rs-fMRI. The rs-fMRI data were preprocessed and calculated to obtain whole brain ReHo values with DPARSF software. The raw ReHo values were subjected to Student’s t test of 2 independent samples with SPM8 software. Results Compared with controls, abnormal SNA in several brain regions related to cognitive control and emotional processing were found in the migraine patients during interictal period, including the frontal lobe, temporal lobe and parietal lobe, etc. ReHo increase was observed in the bilateral anterior cingulate cortex, bilateral rectus gyrus, right precuneus, right posterior cingulate cortex, right inferior parietal lobule, right middle temporal gyrus, and right superior temporal gyrus (P〈0.01), and ReHo decrease was observed in the bilateral supplementary motor area, double side paracentral lobule, right medial frontal gyrus, left dorsal anterior cingulate cortex, and left limbic lobe (P〈0.01). Conclusion Abnormal resting-state functional connectivity exists in several interactions in brain regions of the brain network among migraine patients during interictal period. These regions are associated with cognitive control and emotional processing.
