目的探讨乳腺小肿块超声BI-RADS指标特征与弹性超声结合对早期乳腺癌诊断的影响。方法对手术病理证实的153个最大径≤10mm的乳腺小肿块的超声BI-RADS征象进行分类,探讨各超声征象和病理之间的关系。并对其中128个的弹性图像以三分法进行评分,用两种方法对BI-RADS分类进行调整,比较两种方法的诊断效能。结果常规超声BI-RADS多项指标特征在乳腺小肿块良恶性间的差异有统计学意义(P〈0.05)。弹性对乳腺小肿块的定性诊断灵敏度为89.9%、特异度为77.8%、准确率为85.6%。根据超声弹性对BI-RADS分类进行调整,两种调整方法结果显示方法一较方法二诊断效能更高。结论常规超声BI-RADS分类结合弹性超声可提高对乳腺小肿块的诊断能力。
Objective To discuss the characteristics of BI-RADS in small breast lesions, by combining with ul- trasonic elasticity,to improve the diagnostic rate of early breast cancer. Methods 153 small breast lesions confirmed by operation (maximum diameter≤10 mm), and to evaluate the relationship between the ultrasonographic features in BI-RADS and pathology. Among them there are 128 breast lesions of ultrasonic elasticity, with three points as the standard score, and adjust the ultrasonic BI RADS classification with two methods and compared the diagnostic efficiency. Results There are significant differences in multinomial index feature of BI-RADS in small breast lesions. The sensitivity of elasticity qualitative diagnosis in small breast lesions is 89.9%, the specificity is 77.8%, the accuracy is 85.6 %. Conventional ultrasonic BI-RADS classification adjustment is based on the ultrasonic elasticity, and the results show that the first method is more efficient than the second one. Conclusions The conventional ultrasound BI-RADS classification, combined with ultrasonic elasticity can improve the diagnosis ability of the small breast lesions.
