By investigating the majority of the related Chinese articles and representative English articles in the last five years, this review makes a brief description of severe hypospadias urethroplasties and a detailed analysis of their advantages and disadvantages. Finally, intended patients of different urethroplasties are discussed according to the analytical results.Severe hypospadias urethroplasties vary dramatically, with respective advantages and disadvantages. Duckett combined Duplay urethroplasty is applied extensively in China, others including Koyanagi urethroplasty, onlay island flap, free grafts and staged urethroplasties.There is lack of consistency in how to choose the most suitable severe hypospadias urethroplasfies, especially between one-stage and staged urethroplasties. Cautious decision should be made based on the conditions of patients and the proficiency in skill of surgeons.
