两种不同示踪剂在乳腺癌腋窝前哨淋巴结活检中的应用及对照评价

〔目的〕探讨亚甲蓝与纳米碳2种不同示踪剂应用于乳腺癌腋窝前哨淋巴结活检中的临床价值。〔方法〕选取2013年2月至2015年6月收治入我院进行治疗的120例乳腺癌患者为研究对象,根据采用的示踪剂不同分为A组和B组,A组采用亚甲蓝示踪剂,B组采用纳米碳示踪剂,比较2组患者的检出率与寻找前哨淋巴结所花时间。〔结果〕A组前哨淋巴结检出率为86.6%,准确率为83.33%,灵敏度为92.85%,低于B组的93.33%、88.33%、93.75%,差异不大(P〉0.05);B组寻找前哨淋巴结所花平均时间为(23.63±4.11)min,短于A组的(25.21±6.34)min,差异不大(P〉0.05)。〔结论〕纳米碳与亚甲蓝示踪剂应用于乳腺癌腋窝前哨淋巴结活检中,均具有较好的检出率,且安全性较高,但纳米碳示踪剂对淋巴管和淋巴结的染色效果更清晰,可缩短寻找前哨淋巴结所花时间,值得临床应用与推广。

英文摘要:

〔Objective〕To explore clinical value of two different tracers including the methylene blue and nano carbon applied in axillary sentinel lymph node biopsy of patients with breast carcinoma. 〔Methods〕One hundred and twenty patients with breast carcinoma treated in the Hospital from February 2013 to June 2015 were selected as research subjects, who were divided into Group A and Group B according to different tracers used, patients in Group A were applied with methylene blue tracer, and patients in Group B were applied with nanometer carbon tracer, and the detection rate and the duration to seek the sentinel lymph nodes of patients in two groups were compared. 〔Results〕The detection rate, accuracy and sensitivity of sentinel lymph node in Group A hit 86. 6%, 83. 33% and 92. 85%, being lower than those indexes of patients in Group B at 93. 33%, 88. 33% and93. 75% respectively, and the difference satisfied P 〉0. 05. Average duration to seek sentinel lymph node hit( 23. 63 ± 4. 11)min, being shorter than that of patients in Group A( 25. 21 ± 6. 34) min, but the difference was not significant( P〉0. 05).〔Conclusion〕Both nano carbon and methylene blue tracers applied in axillary sentinel lymph node biopsy patients with breast carcinoma show high detection rate, and safety, but nano carbon tracer casts clearer dyeing effect on lymphatics and lymph nodes, and it can shorten the time to seek sentinel lymph node, worthy of clinical application and promotion.
