目的探讨磁共振动态(DCE-MRI)增强扫描乳腺癌形态学、血流动力学参数及表观扩散系数与分子生物学标记物的相关性研究。方法收集我院2013年6月-2016年12月诊治怀疑乳腺病变的112例患者的临床资料,总结乳腺癌患者DCE-MRI对乳腺癌形态学、血流动力学参数及表观扩散系数,分析DCE-MRI诊断乳腺癌形态学、血流动力学参数及表观扩散系数与分子生物学标记物的相关性。结果 45例乳腺癌患者中,ER、PR、Cerb B-2、p53阳性表达率分别为51.11%、55.56%、46.67%、62.22%。其中ER癌细胞阳性表达与乳腺癌毛刺状边缘呈正相关性(r=0.410,P〈0.05);乳腺癌不均匀强化与Cerb B-2、p53癌细胞表达呈现正相关性(r=0.473,0.212,P〈0.05);Spearman相关性分析结果显示,峰值时间与Cerb B-2阳性表达呈现负相关性(r=-0.381,P〈0.05),ADC值与p53表达呈负相关性(r=-0.346,P〈0.05)。结论 DCE-MRI乳腺癌形态学、血流动力学参数及表观扩散系数与分子生物学标记物具有一定相关性,可根据DCE-MRI图像对乳腺癌患者预后进行有效评估。
Objective To investigate the correlations between morphological,hemodynamic parameters and apparent diffusion coefficient of DCE-MRI in breast cancer on one hand and molecular biology markers on the other hand.Methods The clinical data of 112 patients with suspected breast lesions admitted to our hospital between June 2013 and December 2016 was collected.The histological and hemodynamic parameters and apparent diffusion coefficients of DCE-MRI in breast cancer patients were recorded.The correlations between morphological,hemodynamic parameters and apparent diffusion coefficients of DCE-MRI breast cancer were analyzed.Results The positive rates of ER,PR,Cerb B-2 and p53 were 51. 11%,55. 56%,46. 67% and 62. 22% respectively in 45 patients with breast cancer.The positive expression of ER cancer cells was positively correlated with the burr margin of breast cancer( r = 0. 410,P 0. 05). A positive correlation was detected between the nonuniform enhancement of breast cancer and the expression of Cerb B-2 and p53 cancer cells( r = 0. 473,0. 212,P 0. 05). Spearman correlation analysis showed that the peak time was negatively correlated with the positive expression of Cerb B-2( r =-0. 381,P〈0.05),and that. ADC value was negatively correlated with p53 expression( r =-0. 346,P 0. 05). Conclusion The morphological,hemodynamic parameters and apparent diffusion coefficients of DCE-MRI are correlated with molecular biomarkers.The prognosis of breast cancer patients can be evaluated effectively according to DCE-MRI images.
