目的将e-THRIVE序列重建的对比增强肝门静脉系统血管图像和B-TFE序列采集的非对比增强肝门静脉系统血管图像进行比较分析,为1.5T磁共振肝实质动态增强与肝门静脉成像一站式检查的序列选择提供参考依据。方法搜集60例(每一例均完成e-THRIVE序列对比增强肝门静脉成像及B-TFE序列非对比增强肝门静脉成像)病历资料完整的住院患者磁共振血管影像资料,由两位副主任医师分别对同一病例的两组血管图像评分,对得分情况进行Wilcoxon符号秩检验。结果 e-THRIVE序列组无论是评价肝门静脉系统的肝内部分还是肝外部分均优于B-TFE序列组(P肝内〈0.01;P肝外〈0.01),对肝内部分评价时e-THRIVE序列组的结果客观性更强,对肠系膜下静脉的显示e-THRIVE序列组明显优于B-TFE序列组。结论 1.5T磁共振肝实质动态增强与肝门静脉成像一站式检查通过适当调整检查技术参数,直接利用肝实质动态增强时e-THRIVE序列采集的相关原始数据进行对比增强肝门静脉成像,比在肝实质动态增强前另加一个B-TFE序列进行非对比增强肝门静脉成像更好。
Objective Provide references to magnetic resonance portography and dynamic contrast-enhancement of the liver in one station examination with 1.5T MR by contrast analysis of contrast-enhanced images of hepatic portal venous system vessels after e-THRIVE sequence reconstruction and non-contrast-enhanced images of hepatic portal venous system vessels from B-TFE sequence acquisition. Methods Data of magnetic resonance images of 60 hospitalized patients with complete medical records (all patients have both contrast-enhanced images of hepatic portal venous system vessels after e-THRIVE sequence reconstruction and non-contrast-enhanced images of hepatic portal venous system vessels from B-TFE sequence acquisition) were collected. Vascular images of the same case were scored separately by two deputy directors of physicians. Wilcoxon signed rank test of the scores was done. Results Images of e-THRIVE sequence group have better scores than images of B-TFE sequence group in evaluating both intra-hepatic and extra-hepatic venous system (Pintra-hepatic0.01; Pextra-hepatic0.01). Images of e-THRIVE sequence group have higher objectivity in scores of evaluating intra-hepatic venous system. Images of e-THRIVE sequence group have clear advantage over those of B-TFE sequence group in the displays of inferior mesenteric veins. Conclusion Technical parameters can be adjusted in magnetic resonance portography and dynamic contrast-enhancement of the liver in one station examination with 1.5T MR. After such adjustment, the images of contrast-enhanced hepatic portal veins using relevant raw data collected from e-THRIVE sequence group in dynamic contrast-enhanced liver parenchyma are better than those of non-contrast-enhanced images of hepatic portal venous system vessels from B-TFE sequence acquisition.
