目的 探讨有创机械通气联合丙泊酚、肌松药治疗急性心梗并泵衰竭的临床效果。方法 选取2014年6月-2016年8月于我院ICU接受治疗的48例急性心梗并泵衰竭患者作为研究对象,将采用有创机械通气联用丙泊酚、阿曲库铵进行治疗的24例患者设置为观察组,因某种原因仅使用常规药物进行治疗的24例患者设置为对照组,比较两组间心率、无创平均动脉压（MAP）、血气分析等其他指标、肺部啰音等体征变化以及7 d心脏超声动态图左心室射血分数（LVEF）、平均住院时间变化。结果 观察组患者经治疗后病情明显好转,呼吸频率减慢,心率明显下降,肺部啰音明显减少至消失,低氧血症迅速纠正。对照组患者各项指标也有所改善,但效果明显低于观察组（P〈0.05）,观察组平均住院时间为（20.5±4.9）d,对照组为（36.6±10.3）d,差异有统计学意义（P〈0.05）。观察组不良反应发生率明显低于对照组（P〈0.05）。结论 有创机械通气联合丙泊酚、肌松药治疗急性心梗并泵衰竭效果优于常规药物治疗,有助于提高患者生存率,减少平均住院时间,减轻患者和社会的经济负担,在临床治疗上具有推广意义。

英文摘要：

Objective To explore clinical effect of invasive mechanical ventilation combined with Propofol and muscle relaxant in treatment of acute myocardial infarction and pump failure.Methods 48 patients with acute myocardial infarction and pump failure accepted treatments in ICU of our hospital from June 2014 to August 2016 were taken as research objects,24 patients treated with invasive mechanical ventilation combined with Propofol and Atracurium were as observation group,while 24 patients only treated with conventional drugs because some reason as control group.Heart rate,noninyasive mean arterial pressure （MAP）,blood gas analysis and other indicators,pulmonary rales and other signs change,left ventricular ejection fraction （LVEF） on 7 days of cardiac ultrasound dynamic graph and average hospitalization time between two groups were compared.Results The illness of patients after treatment in the observation group was significantly improved,and respiratory frequency was slower, heart rate was significantly decreased,pulmonary rales was significantly decreased or disappeared,hypoxemia was quickly corrected,patients in the control group were also improved,but the effect was significantly lower than that in the observation group （P〈0.05）.Average hospitalization time in the observation group was （20.5±4.9） d,in the control group of （36.6±10.3） d,with a significant difference （P〈0.05）.The incidence of adverse reactions in the observation group was significantly lower than that in the control group （P〈0.05）. Conclusion Effect of invasive mechanical ventilation combined with Propofol,muscle relaxant in treatment of acute myocardial infarction and pump failure is better than that conventional drug treatment,which it is helpful to improve the survival rate of patients,reduce the average hospitalization time,reduce the economic burden of the patients and the society, thus it has the promotion significance in clinical treatment.
