人上颌中切牙全瓷贴面牙体预备量的研究

目的:通过逆向工程和3D打印技术精确测量1例人上颌中切牙行全瓷贴面的牙体预备量和牙体裸露面积。方法:临床收集健康人上颌中切牙1个,采用Micro CT对其进行三维扫描和重建,3D打印方法将该上颌中切牙进行批量复制,然后按照临床标准进行全瓷贴面牙体预备;最后采用逆向工程软件对牙冠预备前后的牙体组织体积、裸露面积等指标进行测量分析。结果:上颌中切牙全瓷贴面牙体预备磨除的牙体组织体积均值为90.05 mm~3;预备后牙冠的牙体裸露面积均值为107.27 mm~2,其中釉质粘结面积为60.84 mm~2,占预备体粘结面积的56.71%。结论:上颌中切牙瓷贴面牙体预备磨除量约占其牙冠硬组织体积的1/3;预备后贴面裸露面积中釉质组织大于牙本质组织,建议应采用釉质粘结系统进行贴面粘结。

英文摘要:

AIM:To quantify the tooth structure reduction and the bonding area of one human maxillary central incisor preparation for ceramic veneers by Micro CT and 3-D printing technology.METHODS:According to the requirements of clinical experiments,one male maxillary central incisor with normal crown shape and without severe abrasion was selected.The tooth was scanned by Micro CT,then reconstructed into 3-D model and copied by 3-D printing method.Afterwards,veneers were prepared by clinical standards.Finally,The reverse engineering software of Mimics and Geomagic Studio were applied to analyze the volume and area of the dental tissue specimen before and after tooth preparation.RESULTS:The volume loss of maxillary central incisor after tooth preparation for veneer was 90.05 mm3 ,and the total bonding area was 107.27 mm2 (the bonding area of enamel was 60.84 mm2 ,56.71%).CON-CLUSION:The tissue loss of maxillary central incisor for porcelain laminate veneer accounts for about 1 /3 of the crown hard -tissue by volume.The mean percentage of enamel exposure following preparation is more than that of den-tine.The use of enamel bonding system during the placement of porcelain veneer is suggested.
