目的:通过磁共振评价青少年颞下颌关节盘移位患者关节镜盘复位后的髁突改建情况。方法:回顾分析2006年1月—2008年12月间因颞下颌关节盘移位而接受关节镜关节盘复位术且随访期超过6个月的26例青少年患者(38侧关节),在术前和随访磁共振图像上对斜矢状位和冠状位的髁突高度、前后径和内外径分别进行定量测量,应用SPSS13.0软件包中的t检验进行统计学分析。结果:患者手术时的平均年龄为17.7岁,术后平均随访间隔期为15个月。根据Wilkes分类,术前7侧关节为IDⅡ期,26侧为Ⅲ期,2侧为Ⅳ期,3侧为Ⅴ期。随访磁共振显示,所有患者关节盘位于正常位置。与术前相比,在斜矢状位上,髁突前后径和高度分别平均增加了0.45mm(P〈0.05)和0.92mm(P〈0.05);而在冠状位上,髁突内外径和高度的变化无显著差异(P〉0.05)。结论:青少年颞下颌关节盘前移位患者在关节镜关节盘复位术后,髁突可出现以增生为主的明显改建,这种改建可能对改善面形有所帮助。
PURPOSE: To evaluate condylar remodelling after arthroscopic disc repositioning with magnetic resonance imaging(MRI) in teenage patients.METHODS: Thirty eight joints in 26 consecutive patients(aged 14-20 years) with anterior disc displacement of TMJ treated with arthroscopic disc repositioning were included from Jan 2006 to Dec 2008.Magnetic resonance imaging(MRI) was obtained before arthroscopy and after at least 6 months of treatment.Visual inspection and metric analysis were performed in the parasagittal and coronal planes shown on the MRIs.Student’s t test was used to assess the changes in condylar height,width and depth after at least 6 months of treatment with SPSS 13.0 software package.RESULTS: The mean age at first visit was 17.7 years.The mean follow-up interval was 15 months.Before operation,7 joints were in stageⅡ of internal derangement,26 in stage Ⅲ,2 in stage Ⅳ,and 3 in stage Ⅴ.Postoperative MRI showed the disc was in normal position in all joints.The average increase of condylar depth and height in parasaggital plane after arthroscopy was 0.45mm and 0.92mm,respectively(P〈0.05).No difference was found for condylar width and height in coronal plane(P〉0.05).CONCLUSION: The results demonstrates that in the teenage patients with anterior disc displacement of TMJ obvious condylar remodelling can be achieved after arthroscopic disc repositioning,which might be helpful for improving facial profile.However,a well-designed prospective study is needed in the future study.
