目的:探讨凉血解毒汤治疗寻常型银屑病血热证的作用机制.方法:30例寻常型银屑病血热证患者和18例健康人为受试对象,其中给予10例中重度患者凉血解毒汤治疗6周,所有受试者分别于治疗前(W0)采集外周血,10例患者治疗6周(W6)再次采集外周血,分离所有治疗前后外周血单个核细胞,提取总RNA进行实时定量PCR检测,分析外周血单一核细胞(PBMC)中Thl7细胞相关因子IL-17A、IL-17F、IL-21、IL-22 mRNA的相对表达量.结果:患者组PBMC中Thl7细胞因子IL-17A、IL-17F、IL-21、IL-22 mRNA相对表达量均显著高于对照组(均P<0.05);凉血解毒汤治疗6周后,患者PBMC中Thl7细胞因子IL-17A、IL-17F、IL-21、IL-22mRNA相对表达量较治疗前均显著降低(均P<0.05).结论:凉血解毒汤可能通过抑制Th17细胞的活化及其效应细胞因子的分泌,下调17A、IL-17F、IL-21、IL-22 mRNA水平,从而有效治疗血热证银屑病.
Objective:To investigate the therapeutic mechanism of Liangxue Jiedu Decoction on blood-heat syndrome of psoriasis vulgaris.Methods:Thirty patients with blood-heat syndrome of psoriasis vulgaris and 18 healthy individuals were enrolled,and 10 patients with moderate to severe psoriasis were treated by Liangxue Jiedu Decoction for a eonsective 6 weeks.Peripheral blood of all subjects were collected before treatment (W0),Peripheral blood of 10 patients who received therapy were collected again after treatment of 6 weeks (W6).Peripheral blood monocytes of W0 and W6 were isolated and the total RNA was extracted for perform ing Quantitative real-time reverse transcriptase polymerase chain reaction (RT-PCR) assay to analyse expres sion levels of Thl7 cytokines (IL-17A,IL-17F,IL-21,IL-22)in peripheral blood mononuclear ceils (PBMCs). Results:The mRNA expression levels of these eytokines in PBMC of patient group were significantly increased compared with that of healthy subjects (all P〈0.05).And the mRNA expression levels of post-treatment were significantly down-regulated compared with that of pre-treatment (allP〈0.05).Conelusion:The study indicates the therapeutic mechanism of Liangxue Jiedu Decoction on psoriasis of blood-heat syndrome was likely asso ciated with a potential down-regulation of Thl7 cytokines (IL-17A,IL-17F,IL-21,IL-22)mRNA level through inhibiting the activation of Thl7 cells and production of effector cytokines.
