目的:探讨血清癌胚抗原(CEA)、糖链抗原(CA)199水平升高在良恶性疾病中的分布特征及其鉴别诊断价值。方法回顾性分析血清CEA与CA199水平均升高的509例确诊患者临床资料,采用SPSS18.0软件对相关数据进行统计学分析。结果509例患者中,良性疾病患者占22.79%,恶性患者占77.21%,且两组间CEA、CA199水平比较差异有统计学意义( P<0.05)。不同良性疾病患者间CEA水平比较差异无统计学意义(P>0.05),肺癌、肠癌、胃癌、乳腺癌患者CEA水平高于胰腺癌患者(P<0.05),肺癌和肝癌患者CA199水平低于胰腺癌患者(P<0.05)。以10 ng/mL、90 U/mL分别作为CEA、CA199的分界值,良恶性疾病患者间二者水平比较差异均有统计学意义(P<0.05)。结论良性疾病可导致血清CEA、CA199水平升高,而CEA≥10 ng/mL或CA199≥90 U/mL对良恶性疾病的鉴别有一定的辅助诊断价值。
Objective To investigate the distribution characteristics and auxiliary diagnostic value of elevated serum CEA and CA199 in patients with benign and malignant diseases .Methods A total of 509 inpatients with both elevated serum CEA and CA199 level were chosen ,and the related data were analyzed retrospectively with SPSS18 .0 .Results In all 509 patients ,malignant tumor patients and benign diseases patients accounted for 77 .21% and 22 .79% respectively ,and the levels of CEA and CA199 both had significantly statistics differences between the two groups (P〈0 .05) .The patients with pancreatic cancer were with lower CEA level compared with those with lung carcinoma ,colorectal cancer ,gastric carcinoma or breast cancer (P〈 0 .05) ,but with higher CA199 level compared with those with lung carcinoma or hepatic carcinoma (P〈0 .05) .When taking 10 ng/mL and 90 U/mL as cut‐off value of CEA and CA199 ,the differences between the two groups emerged (P〈0 .05) .Conclusion Benign diseases could in‐crease serum CEA and CA199 levels .Meanwhile ,CEA≥10 ng/mL or CA199≥90 U/mL might provide valuable information for the differential diagnosis of benign and malignant diseases .
