信息连续性作为卫生服务连续性的一个关键性指标,其在卫生服务连续性中保证了卫生服务提供者之间、卫生服务提供者与患者之间的健康信息的合理流动。采用文献分析、深度访谈以及问卷分析的方法阐述了卫生服务信息连续性的运行机制,即三位一体:宏观层面要加强融资机制及激励机制,中观层面要建立采购、评估及监管机制,微观层面要建立疾病管理,个案管理,多学科团队护理的协同机制。
Information continuity is a key index in the health service continuity to ensure the reasonable flow of health information among health care providers, and among health service providers and patients. This paper adopted literature analysis, interviews and questionnaire analysis to expound the operation mechanism of continuous information of health service,namely,three-in-one mode: the macro level to strengthen the financing mechanism and incentive mechanism, the meso level to establish procurement,evaluation and supervision mechanism,the micro level to establish disease management,case management,coordination mechanism of multidisciplinary team care.
