英文摘要：

Information continuity is a key index in the health service continuity to ensure the reasonable flow of health information among health care providers, and among health service providers and patients. This paper adopted literature analysis, interviews and questionnaire analysis to expound the operation mechanism of continuous information of health service,namely,three-in-one mode： the macro level to strengthen the financing mechanism and incentive mechanism, the meso level to establish procurement,evaluation and supervision mechanism,the micro level to establish disease management,case management,coordination mechanism of multidisciplinary team care.
