目的:研究体素内不规则运动(intravoxelincoherentmotion,IVIM)~共振成像(magneticresonanceimaging,MRI)技术在肝纤维化评估中的应用价值。方法:对41例进展期肝纤维化患者与11例正常对照者(对照组)行IVIM—MRI扫描,测量标准表观弥散系数(standardapparentdiffusioncoefficience,staADC)值、fastADC(fADC)值、slowADC(sADC)值、fractionADC(fraADC)值,并与血清学肝纤维化相关指标行相关性研究。结果:肝纤维化组与对照组间的fraADC值、sADC值差异有统计学意义(P〈0.05),staADC值、fADC值差异无统计学意义。肝纤维化组1谷氨酰转移酶(gamma.glutamyltransferase.GGT)与staADC值、铁蛋白与staADC呈中度负相关性,铁蛋白与sADC呈中、高度负相关性,铁蛋白与fraADC呈中度正相关性:透明质酸与fADC呈负相关性。结论:IVIM.MRI技术作为临床肝纤维化疾病评估手段具有一定价值.
Objective: To study the value of intravoxel incoherent motion (IVIM) MRI technique for evaluation of liver fibrosis. Methods: Forty-one patients with advanced liver fibrosis and 11 healthy volunteers were enrolled and IVIM- MRI scanning was performed. Standard ADC value(staADC), fast ADC value(fADC), slow ADC value (sADC), fraction ADC value (fra ADC)were measured, and correlations between these parameters and serum liver fibrosis indexes were studied. Results: There was statistically significant difference in fraADC value and sADC value between patients and the controls (P〈0.05), and no significant difference was seen in staADC, fADC . Moderate negative correlation was seen between GGT and staADC value, ferritin and staADC value, respectively. Serum ferritin had moderate to high negative correlation with sADC value, and had moderate positive correlation with fraADC value. Hyaluronic acid had negative correlation with fADC value. Conclusions: IVIM-MRI technique could be a useful method for evaluating liver fibrosis clinically.
