目的探讨血清endocan水平与冠状动脉慢血流的相关性。方法该横断面研究共纳入82例冠脉慢血流患者(慢血流组,n=82)及184例健康对照者(对照组,n=184),血清endocan水平采用酶联免疫吸附(ELISA)法测定,冠脉血流速度采用参照心肌梗死溶栓治疗后(thrombolysis in myocardial infarction,TIMI)血流帧数(TFC)测定。比较两组间血清endocan水平的差异,采用多元Logistic回归分析评价冠脉慢血流的独立危险因子,采用Spearman相关分析及多元线性回归分析评价各因素与TIMI-TFC的独立相关性。结果慢血流组患者血清endocan水平较对照组明显升高(1.05[0.60-1.38]vs 0.74[0.52-1.01]ng/ml,P=0.001),多元Logistic回归分析显示血清endocan水平是预测冠脉慢血流的独立危险因子(优势比2.312,95%可信区间1.329-4.019;P=0.003)。Spearman相关分析结果显示,血清endocan水平与冠脉平均TIMI血流帧数(TIMI frame count,TFC)呈正相关(r=0.311,P=0.004)。多元线性回归显示,在排除混杂因素干扰后,血清endocan水平与冠脉平均TFC仍具有独立相关性(t=2.872,P=0.005)。结论血清endocan水平是冠脉慢血流独立预测危险因子,与冠脉慢血流的严重程度存在正相关,这提示endocan可能成为预测冠脉慢血流发生风险的生物标记物。
Objective To assess the correlation between serum endocan levels and coronary slow flow( CSF). Methods A total of82 patients with CSF and 184 healthy controls were enrolled in this cross-section study. Serum endocan levels were determined by enzyme-linked immunosorbent assay( ELISA). Coronary flow rate was assessed using thrombolysis in myocardial infarction( TIMI) frame count( TFC). The serum endocan level was compared between the two groups. Multivariate Logistic regression was performed to identify the independent predictors for CSF. Spearman correlation coefficient and multivariate stepwise linear regression were employed to assess the independent correlation between variables and mean TFC. Results Serum endocan level( ng / ml) was significantly higher in CSF patients than in controls( 1. 05 [0. 60- 1. 38]vs 0. 74[0. 52- 1. 01],P = 0. 001). Multivariate Logistic regression analysis indicated that endocan in serum was the independent predictor of CSF( odds ratio 2. 312,95% CI 1. 329- 4. 019; P = 0. 003). Spearman correlation analysis revealed that serum endocan levels were positively correlated with mean TFC( r = 0. 311,P = 0. 004). Multivariate linear regression revealed that the endocan was still the significant determinant of mean TFC after adjusting for the potential confounders( t = 2. 872,P = 0. 005). Conclusion Serum endocan level is the independent risk factor of CSF and is correlated with the severity of CSFP,which suggests that endocan might be a potential biomarker for CSF prediction and risk stratification.
