目的探讨十全大补汤联合肠内营养支持对肺癌手术患者免疫功能及临床疗效的影响。方法选取2011年3月-2013年5月于我院行肺癌手术的72例肺癌患者作为研究对象,随机分为对照组和观察组,每组各36例,术后2组患者均给予常规早期肠内营养,观察组患者在此基础上给予口服十全大补汤。治疗后对2组患者手术前后血液免疫指标水平变化、治疗后中医证候评分及免疫功能评价结果进行比较分析。结果 12组患者术后第1天血液免疫各项指标水平均较术前明显降低,而在术后第8天升高且明显高于术前水平(P〈0.05);2组间比较显示,观察组患者术后第8天的IgM、CD3+、CD4+及CD4+/CD8+水平均较对照组患者明显升高(P〈0.05);2观察组患者术后第4天和术后第8天的中医证候积分均较对照组患者明显降低(P〈0.05);3与对照组相比,术后观察组免疫功能提高患者明显增加,而免疫功能下降患者明显减少,差异均具有统计学意义(P〈0.05)。结论十全大补汤联合肠内营养支持可有效提高患者免疫功能并改善其临床症状,相比常规肠内营养支持更加有利于患者术后恢复。
OBJECTIVE To discuss the influence of Shiquandabu Decoction together with enteral nutrition for the immune function and clinical curative effect of lung cancer patients after surgery. METHODS 72 patients with lung cancer after surgery from March 2011 to May 2013 in our hospital were chosen as the research objects and were randomly divided into the control group(36 cases) and observation group(36 cases). The patients of two groups were given conventional early postoperative en- teral nutrition and then the patients of observation group were given Shiquandabu Decoction and comparative study was made a- mong the level change of blood immune indexes before and after operation, TCM symptom score after treatment and immune function evaluation of two groups. RESULTS ① the indexes of the blood and immune of patients in both groups were signifi- cantly lower on the first day after the surgery, but on the eighth day, the indexes were significantly higher than those before the surgery(P〈0.05). The comparison between the two groups showed that the scores of IgM, CD3+,CD4+ and CD4+/ CD8+ of the observation group significantly increased than those of the control group(P〈0.05). ②The TCM syndrome score of patients in the observation group in the fourth and eighth day were significantly decreased than those of patients in the con- trol group (P〈0.05). ③ Compared with the control group, the number of the patients who had major immune function im- provement after the surgery increased significantly in the observation group and the number of patients who had the decreasing of immune function decreased and the differences were of statistical importance(P〈0.05). CONCLUSION Shiquandabu De- coction together with enteral nutrition can effectively improve the immune function and clinical symptoms. Compared with con- ventional enteral nutrition, the combination is more beneficial to the postoperative recovery of patients.
