目的:探讨肌骨超声( musculoskeletal ultrasound, MSUS)评估颈椎钩椎关节有无横向增生的效度。方法23例临床诊断为椎动脉型颈椎病( cervical spondylosis of vertebral artery type ,CSA)的患者,均行颈部MSUS及CT三维重建与血管成像(3D-CTA)检查,分别评估钩椎关节有无横向增生。以3D-CTA为金标准,对MSUS与3D-CTA的评估结果进行比较和相关性、一致性分析,计算MSUS评估钩椎关节有无横向增生的灵敏度、特异度等指标。结果 MSUS评估钩椎关节有无横向增生的灵敏度、特异度、假阳性率、假阴性率、阳性预测值、阴性预测值、诊断符合率分别为:86.44%、96.80%、3.20%、13.56%、92.73%、93.80%和93.48%。 MSUS和3D-CTA评估结果比较差异无统计学意义(χ2=0.25,P >0.05),两者存在关联性(χ2=132.52,P<0.05),相关性较好( r=0.65);并具有极好的一致性( k=0.848)。结论 MSUS评估颈椎钩椎关节有无横向增生具有较好的效度。
Objective To investigate the validity of musculoskeletal ultrasound ( MSUS) for assessing cervical Luschka's joint ( UJ) with lateral osteophyte formation .Methods Twenty-three patients clinically diagnosed as arteria vertebral cervical spondylosis (CSA) were examined with both MSUS and 3-dimensional computer tomography angiography (3D-CTA) to assess whether there was a lateral hyperplasia in uncovertebral joint .Use 3D-CTA as the gold standard,the assessment results of MSUS and 3D-CTA were com-pared and the relevance between them were analyzed .The validity of MSUS assessing UJ with lateral osteophyte formation was also cal-culated.Results There was no significant difference between the assessment results of MSUS and 3D-CTA.A good correlation and consistency (Pearson r=0.65,Kappa value=0.848) were found between the two methods .The sensitivity,specificity,positive predic-tive value,negative predictive value and accuracy rate of MSUS were 86.44%,96.80%,3.20%,13.56%,92.73%,93.80% and 93.48%,respectively .Conclusion MSUS has good validity in assessing UJ with lateral osteophyte formation .
