目的研究甘露聚糖肽注射液对胆囊癌术后患者免疫力的影响以及预防术后感染的作用,以期为临床治疗提供参考。方法选取2011年1月~2013年1月人院诊治的胆囊癌手术患者160例,按数字随机表法分为试验组和对照组,每组各80例;试验组患者在术前3d至术后7d应用甘露聚糖肽注射液,对照组则在相同时间段应用等量的葡萄糖注射液,两组患者在术后均常规应用抗菌药物抗感染;观察两组患者手术疗效、术后感染率、炎症因子白介素一2(IL-2)、IL~6、IL一10、肿瘤坏死因子一a(TNF-a)水平、内毒素、T淋巴细胞亚群CD2+、CD2+、CD2+及NK细胞的变化。结果术后7d,试验组II,10、TNF—a及内毒素方面均较术前显著降低(P%0.05),同时II.一2、IL一10显著升高(P〈O.05);对照组仅IL一6与TNF-a显著降低(P〈O.05);与术前相比,试验组术毕各项指标无显著改变;对照组CD2+、CD2+降低、CO2+/cD2+升高、NK细胞降低;术后4d试验组CD2+升高,CDe’和CD2+/CD8’降低,对照组CD2+和CD2+降低、CD2+/CD。。。升高、NK细胞降低;术后7d试验组CD2+、CD2+升高、CD2+、CD2+/CD2+降低、NK细胞升高,对照组各指标无显著改变,与对照组相比,试验组术毕CD2+、CD2+和NK细胞升高;术后4、7d,CD2+、CD2+升高、CD2+降低、NK细胞升高;试验组感染率为7.5%、对照组感染率为22.5%,上述数据差异均有统计学意义(P〈O.05);试验组术后4、7d总有效率分别为30.0%、90.0%,对照组术后4、7d总有效率为27.5%、82.5%,两次比较差异均无统计学意义。结论甘露聚糖肽注射剂在胆囊癌术前应用,可有效提高患者免疫力,降低感染率,同时不干扰手术疗效。
OBJECTIVE To study the influence of mannatide injection on immunities of gallbladder cancer patients and prevention of postoperative infections so as to provide guidance for clinical treatment. METHODS A total of 160 cases of gallbladder cancer, who were treated in the hospital from Jan 2011 to Jan 2013, were enrolled in the study and randomly divided into the experimental group and the control group, with 80 cases in each, then the experimental group was treated with mannatide injection from 3 days before surgery through 7 days after the surgery, while the control group was given the equal amount of glucose, both groups received the conventional use of antibiotics for treatment of infections; the clinical efficacy, incidence of postoperative infections, the changes of levels of inflammatory cytokines such as interleukin-2 (IL-2), interleukin-6 (IL-6), interleukin-10 (IL-10), tumor necrosis factor-a (TNF-a), and the changes of levels of endotoxin , T lymphocyte subsets CD3 + , CD4 + , CDs + and NK cell were observed and compared between the two groups. RESULTS Seven days after surgery, the levels of IL-10, TNF-a, and endotoxin of the experimental group were significantly reduced (P〈0.05), while the levels of IL-2 and IL-10 were significantly elevated (P0.05) ; the levels of IL-6 and TNF-a of the control group were significantly decreased (P%0.05) ; as compared with those before the surgery, the indicators of the experimental group did not change significantly after the surgery; the levels of CD3+ and CD4+ were reduced, the level of CD4+/CD8+ elevated, the level of NK cell decreased. Four days after surgery, the level of CD3+ of the experimental group was elevated, the levels of CDs+ and CD4+/CD8+ decreased; the levels of CD3+ and CD4+ of the control groups were reduced, the level of CD4 +/CD8 + elevated, the level of NK cell decreased.
